| ObjectiveChronic Obstructive Pulmonary Disease(COPD)is a common and multiple disease that seriously harms human health.It currently ranks as the fourth leading cause of death in the world and seriously affects the quality of life of patients.By 2020,rose to third place.Acute Exacerbation of Chronic Obstructive Pulmonary Disease(AECOPD)refers to an acute onset process characterized by worsening of respiratory symptoms in patients beyond daytime variations,and short-term cough,sputum,shortness of breath,and/or wheezing than usual,increased sputum volume,and purulent sputum are the main manifestations.It has now become an important factor in the death of patients with chronic obstructive pulmonary disease,and is also the main reason for the high cost of treatment for patients.Currently,the treatment of AECOPD in the Chinese Expert Consensus for the Diagnosis and Treatment of Acute Exacerbation of Chronic Obstructive Pulmonary Disease(AECOPD)has a stable effect and effectively improves the patient’s symptoms.However,in recent years,a large number of studies have shown that Western medicine is still limited in the treatment of COPD.Compared with Western medicine standardized treatment,Chinese medicine has certain advantages in the application of the disease,not only can reduce the use of antibiotics and hormones,but also enhance the patient’s physical fitness,reduce the frequency of acute attacks,improve labor capacity and quality of daily life,with important Clinical significance.This study is aimed at the evil spirits such as phlegm and heat,and the lung qi stagnation syndrome.It uses phlegm-heat as the core of syndrome differentiation,and according to the theoretical basis of "exterior of the lungand large intestine",on the basis of heat-clearing and reducing phlegm,we add medicine of bowel-relaxing and checking upward adverse flow of the lung to resolve phlegm to purge the fire of lung.In order to cure the bowel,it is intended to treat the lungs.air lifts normal,coughing,wheezing stops.Therefore,the treatment of the syndrome of phlegm-heat obstructing lung is proposed to use Prof.Jiao’s experience of Ma Xing Lou Shi Tang(Ma Xing Shi Gan Tang add Huangqin Gualou)with clearing heat and freeing lung and relaxing bowel,This study investigated the symptoms,signs,and physicochemical indexes of 56 patients with the syndrome of phlegm-heat obstructing lung of AECOPD in the Department of Respiratory Medicine of DZM hospital,and observed and evaluated the effectiveness and safety of Ma Xing Lou Shi Tang in the treatment of AECOPD.This study provides an effective clinical basis for the research of the combined Chinese and Western medicine treatment of COPD.MethodsUsing a simple and random method,56 patients with AECOPD syndrome of phlegm-heat obstructing lung who met the inclusion criteria were randomly divided into experimental group and control group.The control group was in accordance with the Chinese Expert Consensus for Diagnosis and Treatment of Acute Exacerbation of Chronic Obstructive Pulmonary Disease(AECOPD).Western medicine treatment to the standard;the experimental group in addition to the control group treatment program,the traditional Chinese medicine granules Ma Xing Yu Shi Tang,the drug comes from Kang Ren Tang granule pharmacy,daily 1 agent,1 2 bags,sooner or later,Five days is a course of treatment,a total of two courses.Observe the scores of symptoms and signs such as cough,sputum,wheezing,blood routine,lung function,etc.before the patients were enrolled in the group,at the end of the first course of treatment,at the end of the second course of treatment.After the test is completed,the data is entered into the Chinese version of SPSS 21.0 statistical software,and statistical analysis of the data is performed.The statistical results were "P>0.05 for the difference was not statistically significant,P<0.05 for the difference was statistically significant,P<0.01 for the difference was statistically significant" as the criterion.ResultsThere was no statistically significant difference in age,gender,and course of disease between the two groups,and they were comparable.The minimum age was 45 years and the maximum age was 82 years,of whom 94.6%were over 60 years old.There are 30 males and 26 females.There was no significant difference in symptom scores,CRP,neutrophil percentage,and lung function between the two groups before admission.The baseline was consistent and comparable.At the end of the first course of treatment,there was a statistically significant difference between the two groups at the time of the first course of treatment,P=0.047<0.05,and the symptom improvement in the experimental group had certain advantages over the control group.At the end of the second course of treatment,the differences between the groups further increased.P=0.043<0.05,indicating that at the end of the second course of treatment,the overall symptom improvement of the experimental group was better than that of the control group.Both the experimental group and the control group showed significant statistical differences in the improvement of symptoms before and after treatment.In the two groups of patients after the end of the second course of treatment,the CRP comparison between the two groups P = 0.035<0.05,with statistical differences,the experimental group CRP decreased significantly better than the control group.Ma Xing Lou Shi Tang can significantly reduce the CRP of patients with AECOPD syndrome of phlegm-heat obstructing lung to reduce inflammatory reactions.After the second course of treatment,the two groups of patients had a neutrophil percentage and lung function between the two groups with P>0.05,but there was no statistical difference.However,the P value showed a trend of shrinking,although there was no statistical difference,There is still some clinical significance.There was no statistical difference in platelet counts,alanine aminotransferase,aspartate aminotransferase,creatinine and other safety indicators between the two groups before and after treatment,and there was no adverse reaction after treatment,indicating that Ma Xing Lou Shi Tang has better safety.ConclusionMa xing Lou Shi Tang can improve the overall clinical symptoms in patients with the syndrome of phlegm-heat obstructing lung of AECOPD,and reduce the signs of pulmonary rales.It can be used as early as possible to stabilize and improve the efficiency of clinical treatment.And can effectively reduce patients’ CRP,reduce inflammatory reactions,rapid and timely control of infection,reduce the condition.There is a certain clinical efficacy in reducing the percentage of neutrophils and improving lung function,but it lacks statistical basis.No obvious side effects of blood coagulation and liver and kidney. |
