Based On Data Mining Technology, Professor Liao Jiayu Studied The Rule Of Prescription For Coronary Heart Disease

BackgroundProfessor Liao Jiazhen is a famous expert in the integrated medicine area of traditional Chinese medicine(TCM)and western medicine(WM),who led the in-depth study of TCM treatment of cardiovascular diseases under the guidance of Qi and blood theory,which has a great influence on the clinical treatment and experimental research of coronary heart disease(CHD).It is of great significance to dig and inherit professor Liao Jiazhen's academic thoughts.In recent years,data mining technology has become a hot spot in the inheritance of academic experience of famous TCM veteran teran doctors.Based on the mathematical method,it has been widely used in the experience summary,the literature mining,the law of drug use,the prescription screening and so on.It can get more comprehensive,objective and effective research results.Therefore,we take data mining as the core to study professor Liao Jiazhen's academic thoughts on treating CHD deeply.ObjectiveWe studied the academic thoughts of Professor Liao Jiazhen in the treatment of CHD through the integration of literature review,medical record prescription data mining and expert investigation.Methods1.Literature researchThe contents of Professor Liao Jiazhen's treatment of CHD were retrieved from published papers and treatises to preliminarily understand his thoughts about pathogenesis,conventional medication and treatment ideas.2.Clinical researchRelative medical records were collected,of which the medical notes and TCM prescriptions were extracted to construct database,being analysed by unsupervised integrated data mining methods through auxiliary platform of TCM(V2.5).3.Expert inquiryBased on the results of literature research and clinical research,a questionnaire was designed.Professor Liao Jiazhen's colleagues,students and other experts familiar with his academic ideas were invited,whose opinion surveys were conducted through Questionnaire Star electronic questionnaire platform.With the guidance of Qi and blood theory,clinical research(data mining)as the core and literature research and expert investigation as reference,the academic thoughts of Professor Liao Jiazhen's treatment of CHD were finally concluded.Results1.Literature researchProfessor Liao Jiazhen believes that the basic pathogenesis of CHD is Qi deficiency and blood stasis syndrome,Qi deficiency the basis of blood stasis.The position of CHD is heart,with kidney the root and spleen involved.The basic treatment should be tonifying qi and activating blood.The therapeutic ideas include purgation-tonifying,regulating yin and yang,considering the relation of zang-fu,clear discrimination of orders of priority,and so on.2.Clinical researchThis section included 79 CHD patients,all of which were treated by Professor Liao Jia Zhen in Dongzhimen hospital from 1986 to 1990,with a total of 128 prescriptions,including 47 patients with angina pectoris,87 prescriptions,31 patients with myocardial infarction,32 prescriptions,11 patients with heart failure,19 prescriptions,18 paitients with tachyarrhythmia,37 prescriptions,and 8 patients with bradyarrhythmia,14 prescriptions.The statistical results of the syndrome factors showed that pathogenic factors included blood stasis,phlegm,heat,qi stagnation,retained fluid,cold coagulation,yang hyperactivity,among which blood stasis was the main syndrome factor,and the deficiencies included qi deficiency,yin deficiency and yang deficiency,among which qi deficiency was of the most importance.Among all classifications,the frequencies of qi stagnation and yin deficiency of angina,heat of myocardial infarction,qi deficiency,phlegm,retained fluid,cold coagulation and yang hyperactivity of heart failure and yang deficiency of bradyarrhythmia were the most,while that of qi deficiency of myocardiol infarction was the lowest.For treatment,Professor Liao Jiazhen used radix codonopsis and astragalus membranaceus to tonify qi,radix paeoniae,salvia miltiorrhiza,ligusticum chuanxiong and angelica sinensis to activate blood circulation,pinellia and trichosanthes to reduce phlegm,scutellaria,rhubarb and rhizoma coptidis to clear heat,tulip,fructus aurantii,allium scallion,kawa ko and bupleurum to regulate the qi flow,ophiopogon japonicus,radix paeoniae alba,schisandra chinensis,rehmannia glutinosa and radix sophiaerehmannia to nourish yin,psoralen to warm yang,and tuckahoe,leonurus herba and herba lycopi to promote diuresis.The commonly used compatibility modes included tonifying-qi-activating-blood-circulation,reducing-phlegm-activating-blood-circulation,regu--lation-qi-flow-activating-blood-circulation,clearing-heat-activating-blood-circulation and regulating-qi-flow-reducing-phlegm in treating angina,reducing-phlegm-activating-blood-cir--culation,clearing-heat-activating-blood-circulation,clearing-heat-reducing-phlegm and clea--ring-heat-reducing-phlegm-activating-blood-circulation in treating myocardiol infarction,tonifying-qi-activating-blood-circulation,tonifying-qi-reducing-phlegm and reducing-phlegm-activating-blood-circulation in treating heart failure,tonifying-qi-activating-blood-circulation,reducing-phlegm-activating-blood-circulation,regulating-qi-flow-activating-blood-circulation and regulating-qi-flow-reducing-phlegm in treating tachyarrhythmia and warming-yang-activ--ating-blood-circulation in treating bradyarrhythmia.3.Expert inquiry12 experts participated in the inquiry,all of whom were Professor Liao Jiazhen's colleagues or academic successors.The inquiry included questions about Professor Liao Jiazhen's thoughts of the basic pathogenesis of CHD,the rules of drug use,and the accuracy of data mining and document sorting results.Most of the results and conclusions were affirmed by experts.They pointed out the irrationality of some conclusions and gave proper supplements.Conclusions1.The basic syndrome of CHD is qi deficiency and blood stasis,qi deficiency as the base,blood stasis as the superficiality.2.The basic treatment of CHD is tonifying qi and activating blood circulation,paying attention to the effect of qi-tonifying.The basic prescription of tonifying qi and activating blood circulation includes codonopsis,radix astragali,radix salviae miltiorrhizae,radix paeoniae rubra,radix angelicae sinensis,rhizoma chuanxiong and radix curcumae.3.Phlegm is an important pathological factor of CHD,which can produce evil heat.The treatment is reducing phlegm and clearing heat.4.Compared with other Western Medicine Classifications of CHD,the pathogenesis of angina usually includes qi stagnation on the basis of qi deficiency and blood stasis,and the treatments include regulating qi flow on the basis of tonifying qi and activating blood circulation.5.Compared with other Western Medicine Classifications of CHD,the pathogenesis of myocardial infarction usually focuses more on blood stasis,phlegm and heat on the basis of qi deficiency and blood stasis.The treatments focus more on activating blood circulation,reducing phlegm and clear heat on the basis of tonifying qi and activating blood circulation,and less on tonifying,which embodies the characteristics of the rule "curing the mark if it's urgent".6.Compared with other Western Medicine Classifications of CHD,the pathogenesis of heart failure usually includes yang deficiency and retained fluid on the basis of qi deficiency and blood stasis and focuses more on qi deficiency.The treatments include warming yang and promote diuresis on the basis of tonifying qi and activating blood circulation,and focus more on tonifying qi.7.The arrhythmia type includes tachyarrhythmia and bradyarrhythmia.Tachyarrhythmia focuses more on stagnation of phlegm converting into heat and yin deficiency on the basis of qi deficiency and blood stasis,and the treatments usually include clearing heat,reducing phlegm and nourish yin on the basis of tonifying qi and activating blood circulation,while bradyarrhythmia focuses more on yang deficiency on the basis of qi deficiency and blood stasis,and the treatment includes warming yang on the basis of tonifying qi and activating blood circulation.8.The treatment of Professor Liao on CHD has the characteristics of co-ordinating qi and blood,purgation-tonifying,caring about both yin and yang,considering the relation of zang-fu,treating on both body and spirit,clear discrimination of orders of priority,preventing disease from exacerbating,and so on.
