Objective:The essay aims to provide evidences for rational clinical drug use by analysesing the efficacy and safety of Xiyanping Injection for the treatment of bronchitis in children by systematic review and evaluating the pharmacoeconomics of Xiyanping Injection based on Meta-analysis by cost-effect and least cost analyses.Methods:The essay applies evidence-based analysis and systematic review to evaluate the efficacy,safety and pharmacoeconomics of Xiyanping Injection for the treatment of bronchitis in children.Specifically,searching the key terms including Xiyanping Injection,Xiyanping Injection+bronchitis in children and Xiyanping Injection+conventional therapy respectively in the databases,e.g.China National Knowledge Infrastructure(CNKI),China Technology and Technology Journal Database(VIP),WangfangData,China Biomedicine(CBM),PubMed,EMBASE,and MEDLINE,the essay analyses the efficacy and safety of Xiyanping Injection for the treatment of bronchitis in children by Meta-analysis and evaluating the pharmacoeconomics of Xiyanping Injection based on the price information collected from http://www.315.gov.cn/by cost-effect and least cost analyses.Results:Efficacy and safety:By comparing Yanhuning Injection gourp and Xiyanping Injection(for the treatment of acute bronchitis in children)group,the comparison of the overall response rate is RR? 1.34,95%CI:1.26,1.43,P<0.00001 and the incidence of adverse reactions is RR = 0.29,95%CI:0.15,0.55,P=0.0002.Moreover,by comparing Xiyanping Injection+conventional therapy group and conventional therapy alone(for the treatment of bronchiolitis)group,the comparison of the overall response rate is OR = 10.34,95%CI:4.97,20.56,P<0.00001.Furthermore,by comparing Xiyanping Injection group and Ribavirin(for the treatment of bronchiolitis in children)group,the comparison of the overall response rate is OR?4.72,95%CI:3.11,7.18,P<0.00001.Pharmacoeconomics:Cost-effect analysis for compareing Xiyanping Injection with Yanhuning Injection for the treatment of bronchiolitis in children showed that the C/E=123.93 in Xiyanping group and the C/E=39.47inYanhuning Injection group;incremental cost-effect analysis showed that ICER=371.98;and sensitivity analysis showed that the ICER=420.75.Cost-effect analysis for Xiyanping Injection+conventional therapy versus conventional therapy alone for the treatment of bronchiolitis in children showed that the C/E(excluding lost wages)?179.63and the C/E(including lost wages)=179.63 in Xiyanping Injection+conventional therapy group,and the C/E(excluding lost wages)=1220.31 and the C/E(including lost wages)=2037.11 in conventional therapy alone group;sensitivity analysis showed that the C/E(excluding lost wages)=197.59 and the C/E(including lost wages)=197.59 in Xiyanping Injection+conventional therapy group,and the C/E(excluding lost wages)=1220.31 and the C/E(including lost wages)=2037.11 in conventional therapy alone group.Cost-effect analysis for Xiyanping Injection versus Ribavirin for the treatment of bronchiolitis in children showed that the C/E(excluding lost wages)=238.57 and the C/E(including lost wages)=238.57 in Xiyanping Injection group,and the C/E(excluding lost wages)?1231.7 and the C/E(including lost wages)=2053.68 in Ribavirin group;sensitivity analysis showed that the C/E(excluding lost wages)=262.42 and the C/E(including lost wages)=262.42 in Xiyanping Injection group,and the C/E(excluding lost wages)=1231.70 and the C/E(including lost wages)=2053.68 in Ribavirin group.Conclusions:Xiyanping Injection has favorable safety profile in the treatment of bronchitis in children,and is significantly superior in efficacy and pharmacoeconomics.Particularly in the treatment of bronchiolitis in children,Xiyanping Injection could provide higher overall response rate with lower treatment cost,which indicates that it is a superior therapeutic option and promising for clinical studies and application.The relatively poor quality of the articles included in this systematic review,however,has increased the risk of bias in the conclusions of this systematic review.
