Professor Jia Haizhong Used Microcirculation Theory To Treat The Clinical Experience Of Neurosis And Its Mechanism

Background:The essence of neuroses is a kind of mental disorder,mainly manifests as lasting psychological conflicts,with or without physical function disorder,without clear evidence of pathology.The clinical manifestation of Neuroses can be classified as emotional signs,signs of thinking and body signs.Clinical common symptoms included anxiety,depression,insomnia,dreaminess,palpitation,chest congestion,sweat,belching,bowel,diarrhea,frequent urination,impotence,and somatosensory obstacles.The treatment of neuroses is still a challenge due to the decrease of patients’ life quality,the unobvious superiority of western medicine treatments,the side effects of western medicine.Objective:Based on years of theories and clinical practice combined with Chinese and western medicine,Professor Jia Haizhong put forward a new idea that we can treat neuroses from the perspective of improving microcirculation.The purpose of this paper was to summarize the clinical experience on neuroses of Professor Jia Haizhong’s understanding with microcirculation theory and discussions about neuroses’ mechanism,to provide new understanding,thinking of syndrome differentiation and treatment for neuroses.Methods:This paper was divided into two parts.The first part was the literature review,through the literature retrieval,we organised the knowledge of both the modern medicine and Chinese medicine on pathogenesis,pathogenesis,clinical manifestation,diagnosis and treatment of neuroses.Patients with neuroses can manifest as microcirculation disorder.Regulating qi and promoting blood circulation fomulas represented by Xuefu Zhuyu Tang has certain advantages of treating neuroses.Based on this,theories and pharmacological researches on Xuefu Zhuyu Tang for improving microcirculation and treating neuroses were searched and organized.The second part was experience summary.Organizing the experience ang mechanism of Professor Jia in treating neuroses with microcirculation by sorting medical records,studies in clinic and ward,some typical medical records were provided for study and discussion.Results:1.Neuroses is one common clinical disease,the incidence increases year by year.In modern medicine,the etiology mainly derives from cardiac factors,exact pathogenesis theory has not been put forward,mainly concentrated on neuroendocrine theory,the recent studies find that patients with neuroses have abnormal performance in the level of cerebral blood flow and microcirculation,which explains that microcirculation dysfunction may be the important link in the process of neuroses.2.The diagnosis of neuroses is primarily exemplary diagnosis,except organic diseases,mood disorders associated with or without physical dysfunction can be diagnosed with neurosis;Western medicine in treatment of neuroses have certain limitations besides drug side effects.In Chinese medicine aspect,etiology may include inadequate natural endowment,modern disorders,overwork disorders,long work become empty causes such as the foundation,weakness due to chronic disease and so on.Pathogenesis is mostly intermingled deficiency and excess,because of the block of the brain so that the spirit is out of control or nutrition.People treat neuroses from the aspects of liver,heart,kidney,brain and blood stasis based on differentiation.Treatments such as dispersing stagnated liver qi,promoting blood circulation to remove blood stasis,sedative,filling lean pulp treatment have certain effect.In addition to the traditional Chinese medicine,acupuncture,auricular poin,moxibustion,chiropractic and other methods can be choices.3.Xuefu Zhuyu Tang is the typical fomula of Wang Qingren to regulate qi and motivate blood,and it has certain advantage on the treatment of neuroses.Firstly,Xuefu Zhuyu Tang has certain theoretical basis.As recorded in Yilin Gaicuo,the treatments of Xuefu Zhuyu were mostly resulted by neurological disorders.Secondly,based on the moern pharmacology studies,Xuefu Zhuyu Tang could improve hemorheology and microcirculation.The single herb such as Radix rehmanniae,angelica,red paeony root,peach kernel,safflower,bitter orange,twotooth achyranthes root can effect the arteriole,permeability of capillary network and venule in microculation.Reports about the effectivity in Xuefu Zhuyu Tang to treat neuroses are abundant in clinical.4.Professor Jia treats neuroses with microculation theory in the thought of combined Chinese and western based on years of clinical.Professor Jia argues that,in the onset of neurosis,cerebral microcirculation occupies a very important link in the pathogenesis of neuroses.Microcirculation is the place for material exchange,information transfer and energy transfer,and plays a very important role in the human body normal physiological activities.5.Professor Jia puts forward the innovation of microcirculation syndrome differentiation innovatively,and divides into arterial lesions,micro vein lesions,common lesions,and the capillary network lesions.The traditional Chinese medicine syndrome differentiation can be divide into stagnation and blood stasis syndrome,stagnation and blood stasis syndrome with heat syndrome,stagnation and blood stasis syndrome with cold syndrome,stagnation and blood stasis syndrome with deficiency.Above four syndromes are all on the base of Xuefu Zhuyu Tang,according to different syndrome,diferent drugs dose can be used.Stagnation and blood stasis syndrome with heat syndrome,these patients can increase red paeony root.Stagnation and blood stasis syndrome with cold syndrome,these patients can increase Rhizoma ligustici wallichii and Peach kernel.Ginseng and Astragalus membranaceus are ofen used to Stagnation and blood stasis syndrome with deficiency with Xuefu Zhuyu Tang.Baihe Dihuang Tang is added with anxiety and significantly yin deficiency.Ginseng,radix ophiopogonis,fructus schisandrae,cornus,medicinal indianmulberry root,wheat,and dates can be choosed with depression in lower function.Ageratum,ephedra can be choosed with depression in yin excessiveness.Fangji Dihuang Tang or fritillary bulb is also a good choice with manic.6.Finally,a total of 605 patients’ records with neuroses of professor Jia are organised in November 2014 to May 2016 clinics,based on the clinical analysis of different primary symptom such as insomnia,headache,chest pain,bosom frowsty,palpitation,sighing,depression,anxiety,sadness,crying,neurogenic urinary frequency,we find that Xuefu Zhuyu Tang has curative effects for all kinds of syndrome in neuroses.
