Discussion On The Clinical Pharmacy Service Model And The Application Analysis Of The Chinese And Japanese Hospitals

With the increasing use of traditional Chinese medicine,the rational use of traditional Chinese medicine has also been more and more attention,clinical pharmacy will play a important role in the clinical work.At present,China's clinical pharmacy is still in its infancy,lacking of relevant laws and regulations,clinical pharmacists work content and service model is not uniform.This study focus on the clinical practice of pharmacy,analyze the statues quo of clinical pharmacy services,to explore the clinical pharmacy service model for clinical medicine,providing a reference for the further promotion of clinical pharmacy work.Strychnine has shown a good effect in the rheumatism stubborn paralysis,numbness numbness,traumatic pain and ulcers such as pain in the treatment of ulcers.However,it has a large toxicity,the therapeutic dose and poisoning dose close to the clinical risk of adverse reactions and other factors limit the clinical application of Strychnine in a wide range of applications.This study is based on the practice of clinical pharmacy practice,to explore the clinical pharmacy service model,summed up the analysis of relevant information,and retrospectively analyzed the literature on the adverse events of Strychnos,to analyze and dig the pharmacological monitoring points and risk intervention strategies of Strychnine's clinical application.Based on the China-Japanese Friendship Hospital,explore the clinical use characteristics and existing problems,to lay the foundation for the development of Strychnine related clinical pharmacy work.The research of this subject is divided into three parts:discussion on the Mode of Clinical Pharmacy Service in China-Japanese Friendship Hospital;the retrospective analysis of related literature;analysis of Clinical Application of Strychnos Recipe in China-Japanese Friendship Hospital.1.Discussion on clinical pharmacy service modelFor a period of 8 months of clinical practice of traditional Chinese medicine in outpatient pharmacy,hospitalized pharmacy,decoction room,drug control room,Chinese medicine preparation room,TDM laboratory,clinical pharmacy of China-Japan Friendship Hospital.To understand the China-Japanese Friendship Hospital clinical pharmacy service content and work process,combined with the relevant laws and regulations,summed up the analysis of China-Japanese Friendship Hospital clinical pharmacy service model:basic clinical pharmacy services,clinical pharmacy individual service,clinical Chinese medicine related research.Although the service model needs to be improved,but it can provide a reference for the development of clinical pharmacy services.At present,clinical medicine and related laws and regulations to be improved,there is no clear clinical pharmacists in the job definition and division of responsibilities.It has a direct impact on the development of clinical medicine.2.Retrospective analysis of related literatureBy summarizing and analyzing Strychni group and related Chinese medicine,20 recipients and 22 kinds of Chinese patent drugs are collected from the database.A total of 14 articles and 23 cases of effective cases are collected from the database.Through the retrospective analysis,we obtain the pharmacy monitoring points of Strychnos and the intervention measures of clinical pharmacists:(1)patient compliance:by strengthening the knowledge of the publicity and education,to recognize the patients the importance of rational use of traditional Chinese medicine,reduce the phenomenon of patients with their own medication.(2)drug processing:to standard medicine name,indicate whether the processing of herbs,raw Strychnine only recommended for extermal use,processed products can be taken orally,and Strychnine powder is recommended in order to control the amount.(3)special drug group monitoring:fully understand the patient's physiological and pathological conditions before giving medications,strychnine is forbidden for pregnant women,the elderly,children,liver and kidney dysfunction,high blood pressure and cardiac dysfunction with caution.(4)drug use monitoring:for patients with local symptoms,it is recommended for external use to replace the oral use to reduce toxicity.In the treatment of local pain,it is recommended to homogenized with vinegar when for external use to strengthen the drug into the liver by analgesic effect.In the case of the patient's physical condition,it is recommended to send millet wine to the throne,to enhance the effect of drug blood circulation.(5)after treatment to observe the adverse clinical manifestations of adverse events,it is recommended to take a rest after taking the rest,close observation of changes in symptoms for 2 hours.Strychnine has a cumulative effect in body,long-term medication should be avoided in case of cumulative poisoning.3.Analysis on the Use of Strychnos in China-Japanese Friendship HospitalThis study was conducted to analyze the application of Strychni and Strychnos Chinese Medicine in China-Japanese Friendship Hospital,and to understand the basic information,medication information and prescription of Strychnoside and its preparations,and compare outpatient and inpatient use characteristics.In the analysis of the use of Chinese medicine containing passionate,the introduction of the concept of drug use strength,so that the amount of Chinese medicine more intuitive.68 cases of outpatient pedicure medication,27 cases of hospitalized patients are collected,the use of Strychnine powder patients are mainly in the elderly,of which the age of 65 years of age accounted for 57.4%of elderly patients,37.0%.Outpatient doctor's advice in the absence of traditional Chinese medicine syndrome is more serious(14.7%),many doctor's advice only the description of the patient's symptoms,and the existence of excessive use,longer drug and other issues.Hospitalized doctor's advice in the TCM dialectical information is perfect,and with the efficacy of stalking inscriptions consistent,no excessive use of the phenomenon,the use of more abundant,better prognosis of patients.A total of 3 contains Strychnos ingredients are in the use in China-Japan Friendship Hospital,and 1725 doctor's advices are included.Strychnine related Chinese medicine is mainly used in middle-aged patients,the use of departments are more concentrated,and more for the treatment of rheumatism,rheumatoid arthritis and its pain,cancer and other diseases.Super-drug use phenomenon is widespread(29.6%),of which more than 80%of super-dose medication,drug use intensity was significantly higher than the provisions of the DDD value.Containing Strychnine proprietary Chinese medicine clinical use of most no clear diagnosis of Chinese medicine,the instructions on the taboo crowd defined more vague.
