Objective:1.To analyze the distribution characteristics of syndromes and the characteristics of syndromes and pathogenesis in patients with severe community acquired pneumonia.2.To study the distribution of pathogens in severe community-acquired pneumonia,and to explore the relationship between different pathogens and syndromes and pathogenesis.Method:Using forward-looking research methods,according to the inclusion and exclusion criteria included December 1,2016 to February 28,2017 admission to the Beijing University of Traditional Chinese Medicine First Affiliated Hospital of ICU,China-Japan Friendship Hospital Respiratory and Critical Medical Division ICU,the capital Department of Respiratory and Critical Care Medicine,Chaoyang Hospital Affiliated to Medical University,ICU Patients who meet the diagnostic criteria for severe community acquired pneumonia.Records the contents of the general situation,laboratory examination,the main clinical manifestations and other content;the contents of the questionnaire,the establishment of the original database,the use of SAS software analysis,induction of severe community-acquired pneumonia in patients with TCM syndromes;observed different types of pathogens Type,pathogenesis of the impact of evolution.Results:1.Severe community-acquired pneumonia syndrome type of evolution:severe community-acquired pneumonia admission certificate to phlegm and heat obstruct lung syndrome,phlegm dampness lung syndrome,heat trap card-based syndrome.7,14 days syndrome type to qi and yin deficiency syndrome,lung and spleen deficiency syndrome based.Phlegm and heat obstruct lung syndrome is mainly converted into qi and yin deficiency syndrome,heat trap card-based syndrome is easy to change into a hot poison into the business syndrome from the risk,or into a phlegm and heat obstruct lung syndrome out of the qi points to the more.Phlegm dampness lung syndrome easily converted into phlegm and heat obstruct lung syndrome and lung and spleen deficiency syndrome.Qi and yin deficiency can be transformed into lung and spleen deficiency syndrome.2.Severe community-acquired pneumonia syndromes and the relationship between pathogens:severe community-acquired pneumonia to Gram-negative bacteria.Common pathogens for Pseudomonas aeruginosa,Acinetobacter baumannii,Klebsiella pneumoniae and so on.Gram-positive bacteria to Streptococcus pneumoniae,Staphylococcus aureus mainly;viruses,fungi and atypical pathogens can also be infected.There was a correlation between SCAP syndromes distribution between bacteria,fungi,viruses,mycoplasma and each observation point(day 1,day 7,day 14).Gram-positive bacteria and gram-negative bacteria were distributed in each syndrome type No obvious difference.Conclusion:1.Severe community-acquired pneumonia into the ICU to the heat syndrome as the main performance,syndromes to phlegm-heat obstructing lung syndrome mainly to evil heat flow gas-based,but also a hot poison into the blood of the critical syndrome.Heat evil nostalgia gas consumption gas injury caused by Qi and Yin Deficiency.Can be seen in severe community-acquired pneumonia,the core of the pathogenesis of wind and heat poisonous evil body virtual invasion,pulmonary injury,declared off disorders,lung Wei evil evil puzzled,then phlegm stasis poisoning lungs,Trap trap blood,heat trap pericardium,and further Yin off the sun off and died certificate.Rehydration therapy can correct the lack of body fluid.2.Bacterial infection is more prone to phlegm obstruction lung qi card,and virus infection faster transmission,easy to cause heat trap package,heat poisoning into the blood of the more serious syndrome.Keeping good gas is the key to preventing critical illness and reducing mortality.
