The Research On The Mode Of Home-based Care With Combination Of Medical And Elderly Care From The Perspective Of Government Responsibility

As the phenomenon of demographic aging, long-lived, empty-nest and frail elderly of China has been increasing emerging, the problem of long-lived but unhealthy is becoming more and more serious, the current the Mode of Home-based Care can't satisfy the demand of long-term care services for the elderly any longer. Under the background of healthy aging, we advocate to improve the Mode of Home-based Care by combining medical and elderly care, in order to realize a continuous, professional and institutionalized long-term care which contains integrating prevention, rehabilitation care and community home-based care, to maintain or restore the self-care ability of the elderly, extending their healthy life cycle, reducing the possibility of becoming disabled or half the disabled, shortening the time that they can't take care of themselves in their final phases of life.The article, based on the reflect of anti-fall project of S-street, put forward the Mode of Home-based Care with Combination of Medical and Elderly Care, which is aimed at home for in-home elderly. By means of interviewing several persons who are in charge of Civil Administration Department and Health Sector, Community leader and related personnel of Community Health Center, I try to explore the development status of the Mode of Home-based Care with Combination of Medical and Elderly Care and find out the gap between ideal and reality from the perspective of government responsibility, moreover, make emphasis on the integration and utilization of elderly care and medical resources to achieve valid configuration of health care and elderly care, so that promote the development of the Combination of Medical and Elderly Care in the Model of Home-based Care.The author believes that the advance of integrating pension service with medical service in the mode of home-based care for the aged is not simply in accordance with the existing administrative structure embedded in the service content. The civil affairs department is responsible for providing home-based care services, the health department is responsible for providing medical and health services, and "combination" is provided by both of them. Independent administrative structure has irreconcilable contradiction and gambling, even trying to achieve their same goal of the old service, the independent of human rights, powers and financial departments also become a problem in the interaction of implementing the same policy. Therefore, the author considers that straightening out the management system by the top-level design, repositioning system to set out for the elderly needs, eventually providing continuous care for the elderly smoothly in the same system.
