Objective: Exploratory attempts to determine the best Ki-67 expression index of gastric cancer staging and its correlation analysis with clinicopathological features and prognosis of gastric cancer.Methods: Retrospective analysis between December 2014 and December 2016,Qinghai university affiliated hospital treated gastric cancer radical R0 resection in patients with clinical data,the operating characteristic curve(ROC)participants judgment R0 resection of gastric cancer staging the best expression of Ki-67 index.The relationship between the value and the clinicopathological features of gastric cancer was determined using the chi-square test and Fisher's exact probability method.Kaplan-Meier method and Cox proportional risk model were used for survival analysis.Results: General data and clinicopathological data of 185 patients with R0 resection of gastric cancer were collected,including 146 males and 39 females,the age was 56.73±9.579 years old.In this study,there were 3 cases in the 0 stage,49 in I stage,70 in II stage,and 63 in III stage.AJCC(American Joint Committee on Cancer)staging was used to determine the depth of tumor invasion as early gastric cancer or advanced gastric cancer.It is concluded that the best expression index of Ki-67 expression for advanced gastric cancer is 25%,sensitivity is 84.6%,specificity is 56.2%,the area under the curve is 0.702,P = 0.008,95% CI(0.540,0.864),and the Youden index is 0.409.Ki-67?25% and Ki-67>25%,compared in p T stage,tumor grade,clinical stage,vascular invasion and distant metastasis were statistically significant(P < 0.05).There was a positive correlation between the expression index of Ki-67 and the expression of Her-2,and the difference was statistically significant.There was no significant difference in DFS(P =0.148)between the two groups.COX multifactor regression analysis showed that T3,N stage,clinical stage and Lauren type were independent risk factors for the recurrence and metastasis of gastric cancer patients with R0 resection.Conclusion: The best expression index of Ki-67 is 25% in the diagnosis of early and advanced gastric cancer.The cut-off value of the Ki-67 expression index was statistically significant in p T staging,tumor classification,clinical stage,vascular invasion,and distant metastasis.
