Objective To retrospectively compare the demographic, clinicpathological features and prognostic characteristics between male and female gastric cancer(GC)patients. Methods A total of 3937 Chinese patients, including 3098 males(78.7%)and 839females(21.3%),diagnosed with GC at the General Hospital of NingXia Medical University of China were selected through inclusion and exclusion criteria, and the prognosis of the patients was followed up.Clinicopathological features were assessed, and univariate and multivariate survival analyses were performed. Results A total of 3937 patients with gastric cancer were included in the analysis, including 3098 male patients(78.7%)and 839 female patients(21.3%),with a male-to-female ratio of 3.7:1.1.In terms of age composition, the proportion of young(?40 years old)patients with gastric cancer was higher than that of men,the proportion of male and female was equal among middle-aged(41-60 years old)patients with gastric cancer, and the proportion of male patients with gastric cancer in the elderly(>60 years old)was significantly higher than that of fmale.Distribution differences are statistically significant(?~2=63.842,p<0.001).2.Males have a significant quantitative advantage in the history of smoking and drinking as a high risk factor for the onset of gastric cancer. The difference was statistically significant(?2=700.500,p<0.001;?2=234.373,p<0.001).3.In terms of pathological T-stage,the proportion of female patients with gastric cancer in stage(T1-T2)was higher than that in male patients, and the male patients in stage(T3-T4)was higher than that in female patients. The difference was statistically significant(?~2=14.986,p=0.020).4.The proportion of proximal one-third gastric cancer in male patients were higher than that in female patients, and the proportion of distal one-third gastric cancer in female patients were more than that in male patients. The difference was statistically significant(?~2=66.791,p<0.001).5.In the degree of tumor differentiation, the proportion of highly differentiated tumors was higher in male patients, while the proportion of low differentiated gastric cancer in female patients were higher than that in male patients. The difference was statistically significant(?~2=37.293,p<0.001).6.There was no significant difference in distribution of blood type, family history, pathological n-stage,pathological m-stage,borrmann classification, tumor size among male and female gastric cancer patients(p>0.05).7.The difference in 1-year survival(57.9%vs.59.8%),3-year survival(46.1%vs.48.0%)and 5-year survival(22.2%vs.17.0%)in patients with gastric cancer in male and female patients were not statistically significant(p>0.05).8.Among the smoking population, the prognosis of the female patients with smoking was poor(p=0.016),while the male patients with smoking had no significant effect on the prognosis(p=0.704).between the sexes, the mortality of female patients with smoking were significantly higher than that in male patients, with a statistically significant difference(p=0.016).9.The prognosis of male patients with gastric cancer was worse than that of female patients during stage iv(p=0.022).10.Multivariate survival analysis showed that age, tumor differentiation, pathological T-stage,pathological N-stage,clinical stage were independent risk factors for prognosis of gastric cancer patients(p<0.05).Conclusion The proportion of gastric cancer patients in young female was higher than that in male, and the proportion of middle-aged and elderly patients with gastric cancer was lower than that of male. The proportion of poorly differentiated cancers in female was higher than that in male, and the rate of high differentiation gastric cancer in female was lower than that in male.Smoking is an independent prognostic risk factor in women with gastric cancer, but there is no significant correlation between smoking and male prognosis.Sex disparities are not independent prognostic risk factors in gastric cancer population.
