Objective To analyze the clinical characteristics of acute exacerbation of chronic obstructive pulmonary disease?AECOPD?with different peripheral blood eosinophils?EOS?,and to investigate the guiding value of blood EOS in the treatment and prognosis of AECOPD patients. Methods According to the inclusion and exclusion criteria, the clinical data of 108AECOPD patients admitted to the Hospital of Ningxia Medical University from December2014 to December 2016 were retrospectively analyzed. The patients were divided into two groups according to EOS%in blood routine at admission,EOS%?2%for group I?n=28?,and EOS%<2%for group II?n=80?.To compare the two groups in age, gender, smoking history, duration of disease,BMI,complication, blood routine related indicators of admission,CRP,ESR,blood gas analysis, the number and days of intravenous glucocorticoid during hospitalization, daily glucocorticoid dose used, stay in care room, the use of mechanical ventilation, length of hospital stay, and return to the situation at discharge. Results No significant difference was found between the two groups in age, gender, smoking history, duration of disease,BMI,comorbidities, blood gas analysis?PH,PCO2,PO2,SaO2%,HCO3-,BE?,CRP,ESR,daily glucocorticoid dose used and stay in care room?P>0.05?.However,significant difference was found in the blood routine?leukocyte and neutrophil?,the number and days of intravenous glucocorticoid during hospitalization the use of mechanical ventilation, length of hospital stay, and return to the situation at discharge?improvement, hospital death, exacerbation cause auto-discharge??P<0.05?.Conclusion The level of peripheral blood EOS could guide the stratified treatment of patients with AECOPD,and may be used as a biomarker for prognosis of the disease.
