Imaging Studies On The Correlation Between Degenerative Lumbar Spondylolisthesis And Lumbar Joint Morphology

Objective To investigate the correlation between the morphologic changes of the lumbar facet joint and the degenerative spondylolisthesis..Methods Retrospective analysis of spinal Department of orthopedics general hospital from July 2014 ~2018 in Ningxia Medical University in January 40 only L4/5 degenerative spondylolisthesis spondylolisthesis group(DLS group),including 32 women,8 men,age range 46~76 years,mean age 62.49 years;over the same period of lumbago or lumbocrural pain without lumbar spondylolisthesis patients as control one group,29 female,11 male,aged 45~70 years old,the average age of 57.36 years.The age and sex of the 2 groups were matched,and all the X-ray films of the lumbar spine,the lumbar CT and the lumbar MRI were performed.The measurement index of lumbar CT 3D reconstruction are as follows:two groups of patients with L4 vertebral endplate diameter,L4 vertebral height,L4/5 intervertebral height,two groups of patients with L4/5 segment transverse,sagittal,coronal facet joint angle,axial lumbar facet diameter,sagittal facet joint sagittal diameter;lumbar MRI measurement the following indicators: L4/5segment transverse lumbar facet joint articular surface curvature,curvature ratio calculationResults There was no significant difference between the baseline data of age,sex,end plate diameter,intervertebral height and vertebral height(P >0.05).Slippage group and control group in L4/5 segment on both sides of the transverse,sagittal and coronary lumbar vertebra facet joint angle according to age group comparison,there is no obvious difference,no statistically significant difference(P > 0.05);the transverse section and sagittal lumbar joint Angle were larger than the control group on both sides of the L4/5 segment of the slide group,and the difference was statistically significant(P< 0.05).The difference between the two sides of the coronal lumbar facet angle was not statistically significant compared with the control group(P > 0.05);the curvature ratio of the lumbar facet joint in L4/5 segment was greater than that of the control group,the difference was statistically significant(P < 0.05);The transverse and sagittal diameter of the lumbar facet joint in the L4/5 segment of the spondylolisthesis group was greater than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Sagittal lumbar facet joint angle in spondylolisthesis in DLS patients is the result of uneven stress and abnormal stress in the process of lumbar facet joint degeneration,and may be the inducement of degenerative lumbar spondylolisthesis;LFJ joint surface matching degree was lower in DLS patients.The area of facet joint of DLS patients increased.
