Objective To evaluate the efficacy and safety of photodynamic therapy(PDT)combined with local closure in the treatment of moderate to severe acne and to prove its clinical utility.Methods A total of 91 patients with moderate to sev ere acne were divided into three groups randomly.31 patients in the combination-therapy group were given 5% ALA-PDT with 3 times for 1 courses.For patients with nodules or cysts larger than 1cm,triamcinolone acetonide combined with gentamycin sulfate an d lidocaine were locally blocked first.30 patients in the ALA-PDT group were only given 5% ALA-PDT with 3 times.30 patients in the drug group take minocycline capsules orally 100 mg a time,twice a day,for 8 weeks.At the 2nd,4th,6th,8th,16 th week,the efficacy and adverse reactions were evaluated.Result(1)At the 6th week,the effective rate of ALA-PDT combined with local closure group was better than other groups,the differences were statistically significant(P< 0.05).At the 16 th week,the effective rate of ALA-PDT combined with local closure group were better than drug group,the differences were statistically significant(P< 0.05),but compared with ALA-PDT group,there was no significant difference(P> 0.05).(2)At the 6th,8th,16 th week,the cure rate of PDT combined with local closure group and ALA-PDT group were better than the drug group,the difference was statistically significant(P< 0.05),in the 16 th weeks,the cure rate of ALA-PDT combined with local closure group was better than other groups,the differences were statistically significant(P< 0.05).(3)The adverse reaction rate in the drug groups was higher than other groups,the differences were statistically significant(P < 0.05).Conclusions(1)Three therapies all achieve good effects in the treatment of moderate to severe acne.(2)Among the three groups,there is a statistically significant difference in the cure rate,the treatment with ALA-PDT combined with local blocking is best,drug therapy was poor.(3)The ALA-PDT combined with local closure therapy has a few adverse reaction and long term cosmetic effect. |
