Background and Objective:According to anatomical site gastric cancer can be divided into proximal and distal.In western countries,the overall incidence of gastric cancer has declined over the past 50 years,This is mainly due to a significant reduction in the incidence of distal gastric cancer,However,the incidence of proximal gastric cancer,especially cardiac cancer,has been increasing in recent years.Gastric cancer has a tendency toward proximal develop.The study found that proximal gastric cancer,especially the cardia cancer,had distinct biological and pathological characteristics with distal gastric cancer.Therefore,to understand the clinical pathological characteristics of gastric cancer in different site,In the future,we can better diagnose and treat gastric cancer in different site.Methods:To collect gastric cancer patients who were hospitalized in the first hospital of lanzhou university from 2012.01 to 2016.12,A total of 370 cases(141 cases of proximal gastric cancer,229 cases of distal gastric cancer),all patients were diagnosed as gastric cancer by pathological biopsy after surgery.Compare and analyse age,sex,smoking,drinking,body mass index,blood lipid,tumor markers,tumor site,size,pathological type,gross type,lymph node metastasis,TNM staging,and analyse of the correlation between blood lipid level with TNM stage of tumor.Using SPSS 21.0 statistical software package for data analysis,p<0.05 was considered significant.Results:1.In 370 patients with gastric cancer,the male incidence of proximal gastric cancer were higher than in distal gastric cancer,patients with onset age group showed that the incidence of proximal gastric cancer is mainly in the elderly,and the incidence of distal gastric cancer is more common in middle-aged and young people.The differences were statistically significant(P<0.05).2.Overweight and obesity in the proximal gastric cancer were higher than the distal gastric cancer the differences was statistically significant(P<0.05).3.Proximal and distal gastric cancer had no statistical significance in smoking,drinking history,tumor size,tumor markers,and lymph node metastasis(P>0.05).4.In the TNM staging of proximal gastric cancer,stage III and IV were more common than distal gastric cancer(P<0.05).5.There is no significant difference in lymph node metastasis between proximal and distal gastric cancer(P>0.05).6.According to Spearman correlation analysis showed that serum lipids(TC,TG,LDL)were negatively correlated with TNM staging.Conclusion:1.Proximal gastric cancer is mainly older men,overweight and obese patients are also more than distal gastric cancer,and its tumor differentiation was worse than distal gastric cancer,and TNM stage were also later.2.The lack of specific clinical symptoms in the early stage of gastric cancer,most of them were found in the middle and late stage,so the detection rate of early gastric cancer was significantly lower than that of distal gastric cancer.3.In recent years,the admission rate of proximal gastric cancer in our hospital has gradually increased,but the admission rate of distal gastric cancer has gradually declined.
