Objective: To investigate if there is prognostic difference between proximal gastric cancer and distal gastric cancer The relationship between pronostic difference and clinicopathological characteristics surgical treatment is analyzed Methods: There are 155 patients underwent operation for proximal gastric cancer and distal gastric cancer (39 patients with proximal gastric cancer, 116 patients with distal gastric cancer) from July 1999 to December 2001 Clinical data were recorded, survival data were obtained from follow-up Prognostic difference is compared The relationship between prognostic difference and clinicopathological characteristics, surgical treatment is analyzed Results: The rate of follow-up of the patients is 70.0% (109/155) 25 patients had a proximal gastric cancer, 84 patients had a distal gastric cancer There were no significant difference in age and sex between proximal gastric cancer group and distal gastric cancer group There is no significant difference in histological classification between two groups The depth of tumor infiltration in proximal gastric cancer group is deeper than that in distal gastric cancer group, the difference has statistical significance There is no significant difference in TNM stages between two groups The rates of radical resection have no significant difference between two groups The rates of positive surgical margin postoperative morbidity and combined resection of invaded organs of proximal gastric cancer group is higher than that in distal gastric cancer group Even though the 5-year survival rate and median survival dates in proximal gastric cancer group is lower than that in distal gastric cancer group (23.9 月 VS33.3 月 , 24.4%VS42.1%), but the difference has no statistical significance (P=0.1195 ) Conclusions: The incidence of proximal gastric cancer is rising in the western countries Proximal gastric cancer draws more and more concern from clinicians...
