Objective To analyze the clinical characteristics of patients with polycystic ovary syndrome(PCOS),to investigate the related risk factors of PCOS patients,and to establish a risk assessment model for PCOS.Methods A questionnaire was used to investigate PCOS patients and their control group.In this study,131 cases of patients who were diagnosed with polycystic ovary syndrome at the Reproductive Medicine Center of the Second Hospital of Lanzhou University from June 2014 to August 2017 were selected as case group.130 cases of non-PCOS patients who visited to obstetrics and gynecology clinic and had regular menstruation and normal fertility were selected as control group 1(internal control group)and 265 cases of general population of reproductive age in community who had regular menstruation with non-PCOS were selected as control group 2(external control group),a total of 526 people were investigated.According to Rotterdam diagnostic criteria of polycystic ovary syndrome,the clinical and metabolic characteristics of PCOS patients were retrospectively analyzed,and the risk factors of PCOS were analyzed by case-control study.Epi Data3.1 software was used to data input,SPSS21.0 software was used to analyze data,statistics results were showed in mean ± standard deviation,P<0.05 indicates that the data was statistically significant.Non-conditional logistic regression was used to analyze related risk factors of PCOS,and established a logistic regression model to analyze the related risk factors of PCOS.Results(1)The clinical characteristics and metabolic characteristics of patients with polycystic ovary syndrome: 131 PCOS patients average age was 25.08±3.85 years,of which about 50% patients with BMI more than 23kg/m2,about 26% of patients showed obesity.The menstrual features of those PCOS patients are mainly characterized by a disorder or prolonged menstrual cycle,about of 60% patients have a menstrual cycle of approximately 90 days.According to the ESHRE/ASRM classification criteria,in 131 PCOS patients,type I O+H+P accounted for approximately 28.2%,type II H+P accounted for about 12.2%,type III O+P accounted for about 58.1%,and type IV O+H accounted for about 1.5%.In different PCOS phenotype the age of menarche was statistically significant(P <0.05).In different age groups,PCOS patients less than 20 years old mainly exhibited insulin resistance and hyperandrogenism,while with the increasing of age,the proportion of patients with insulin resistance and hyperandrogenism is decreased.In normal glucose tolerance group and impaired glucose tolerance group in PCOS patients,impaired glucose tolerance PCOS patients most showed obesity and insulin resistance.In obese and non-obese PCOS patients,differences(P <0.05)were existed in HOMA-IR,2 hours postprandial blood glucose,2 hours postprandial insulin.In insulin resistance and non-insulin resistance PCOS patients,BMI was significantly difference(P <0.05).(2)Univariate analysis results of PCOS influence factors: The results showed that the incidence of PCOS is related with low household income,BMI,the time from menarche to establish a regular menstrual cycle,mother's nutrition during pregnancy,family history of hypertension,obesity and PCOS in first-degree relatives,low birth weight,long-term passive smoking,exercise,stress,and environmental endocrine disrupting chemicals(EDCs)factors(P<0.05).(3)Multivariate Logistic regression analysis results of PCOS influence factors: According to the results of univariate analysis,we selected meaningful factors into the multivariate Logistic regression analysis.The results showed that BMI more than 25kg/m2,the time from menarche to establish a regular menstrual cycle more than 2 years,low birth weight,EDCs factors such as frequently use disposable plastic tableware or plastic cups,often contact with kitchen fumes,residence or workplace decoration history,residences around petrochemical plant or waste incineration point are risk factors for PCOS,while exercise regularly,well-nourished during pregnancy are protection factors.(4)Establishment of PCOS risk factors Logistic regression model: Logit P=-16.085+0.267X2 + 1.601X3-0.095X4 + 1.620X17 + 1.008X7-1.671X9 + 1.21X12 +2.413X14+2.9X15.Conclusions In this study,reproductive age women with POCS mainly exhibited hyperandrogenism and insulin resistance,impaired glucose tolerance PCOS patients most showed obesity and insulin resistance.A simple Logistic regression model for risk factors of PCOS was initially constructed.
