Characteristics Of Different TCM Syndromes Of Polycystic Ovary Syndrome And Its Correlation With Clinical Indicators

Objective:Through the collection and analysis of TCM syndrome differentiation and clinical indexes of 1059 cases of Polycystic Ovary Syndrome(PCOS),the distribution characteristics of different TCM Syndrome Types and the relationship with clinical indexes were studied,and the distribution rules and clinical characteristics of different TCM Syndromes of PCOS were revealed in order to be PCOS it provides an objective basis for the differentiation and treatment of syndrome differentiation and treatment.Methods:A retrospective analysis was made for the patients with polycystic ovary syndrome(PCOS)in the Department of gynaecology of the First Affiliated Hospital of Heilongjiang University Of Chinese Medicine from May 2013 to November 2017.1059 cases of PCOS patients were collected in accordance with the inclusion criteria,and their basic information and related examination results were arranged.According to the PCOS diagnosis and treatment scheme of the national clinical research base and the TCM syndrome differentiation standard in the path,combined with the TCM syndrome grading scale and the information of traditional Chinese medicine,the PCOS patients were differentiated into spleen deficiency phlegm dampness syndrome,kidney deficiency liver depression syndrome,kidney deficiency blood stasis syndrome and phlegm stasis syndrome,and the distribution and clinical characteristics of the four syndromes were analyzed.Come to conclusion?Results:(1)The proportion of the TCM Syndrome Types in PCOS was 41.45%(439 cases),28.99%(307 cases),19.26%(204 cases)and 10.29%(109 cases)with kidney deficiency and blood stasis syndrome,of which spleen deficiency phlegm dampness syndrome was the main syndrome type.(2)The age of PCOS in the syndrome of kidney deficiency and stagnation of liver is greater than that of spleen deficiency and phlegm dampness syndrome and phlegm and stasis syndrome,the difference is statistically significant(P<0.05);The incidence of DPB,SBP,weight,BMI,WC,HC,WHR,black spiny skin and spillover of PCOS patients with spleen deficiency syndrome and phlegm stasis syndrome is significantly higher than that of kidney deficiency and kidney deficiency syndrome and kidney deficiency and blood stasis syndrome,the difference was statistically significant(P<0.05);The incidence of hairy in PCOS patients with phlegm and stasis syndrome and kidney deficiency syndrome was significantly higher than that of spleen deficiency syndrome of phlegm dampness and kidney deficiency and blood stasis(P<0.05);The incidence of acne in patients with PCOS of phlegm and stasis syndrome and kidney deficiency and blood stasis was significantly higher than that of spleen deficiency phlegm dampness syndrome and kidney deficiency liver depression syndrome.The.difference was statistically significant(P<0.05).(3)The LH,LH/FSH,PRL and SHBG of PCOS patients with kidney deficiency and kidney deficiency syndrome were significantly higher than those of spleen deficiency syndrome of phlegm dampness and phlegm stasis,and the difference was statistically significant(P<0.05);The FSH level of PCOS patients with kidney deficiency syndrome of liver Qi was higher than that of phlegm and stasis syndrome(P<0.05),and the T of spleen deficiency phlegm damp syndrome PCOS patients T The difference was statistically significant(P<0.05).The AND of PCOS patients with phlegm and stasis syndrome was significantly higher than that of spleen deficiency syndrome of phlegm dampness,kidney deficiency liver depression and kidney deficiency and blood stasis(P<0.05);The FAI of PCOS patients with spleen deficiency syndrome of phlegm dampness and phlegm stasis syndrome was significantly higher than that of kidney deficiency liver.The difference between depression syndrome and kidney deficiency and blood stasis syndrome was statistically significant(P<0.05).(4)The level of FPG,FINS,HOMA-IR,TC,TG,LDL-C,Apo-B,Apo-B/Al of spleen deficiency phlegm dampness syndrome and phlegm stasis syndrome were higher than that of kidney deficiency and kidney deficiency syndrome and kidney deficiency and blood stasis syndrome,the difference was statistically significant(P<0.05),of which the spleen deficiency phlegm dampness syndrome was the highest in PCOS patients.The levels of TG,LDL-C,Apo-B andApo-B/Al were the highest.The levels of HDL-C and Apo-Al in PCOS patients with kidney deficiency,liver depression and kidney deficiency and blood stasis were higher than those of spleen deficiency,phlegm dampness syndrome,phlegm stasis syndrome,and the difference was statistically significant(P<0.05).(5)Spleen-deficiency phlegm-dampness syndrome was positively correlated with FAI,FINS,FPG,HOMA-IR,TC,Apo-Al and LDL-C,and negatively correlated with hairy,TG and HDL-C(P<0.05).Kidney deficiency and liver depression were negatively correlated with DBP,FPG and TG(P<0.05).The syndrome of kidney deficiency and blood stasis was positively correlated with hypertrichosis,acne,FPG and FINS,and negatively correlated with WC,acanthosis nigricans and HOMA-IR(P<0.05).Phlegm stasis syndrome was positively correlated with WHR,hairy,FPG,FINS,HOMA-IR,LDL-C and Apo-B,and negatively correlated with FSH,TC,Apo-Al and Apo-B/Al(P<0.05).Conclusions:(1)The TCM Syndromes of PCOS patients are mainly spleen deficiency and phlegm dampness syndrome.(2)The age of the menarche of the syndrome of kidney deficiency liver depression and kidney deficiency and blood stasis is relatively large,with abnormal sex hormone metabolism as its main clinical feature.The main clinical features of PCOS patients with spleen deficiency syndrome of phlegm dampness and phlegm and blood stasis are abdominal obesity,androgen increase and glucose and lipid metabolism disorder.(3)The TCM syndrome type and the typical external signs of polycystic ovary syndrome and glycolipid metabolism indexes have obvious correlation,which provides an objective basis for the standardized diagnosis and treatment of PCOS syndrome type.
