Objective: To study the effect of duloxetine concentration on the core symptoms and somatic symptoms of Depression.Further clarifing the relationship between the concentration of different blood drugs and the different target symptoms of depression in patients with depression.To provide more rational drug regimen for individualized treatment,improving the efficacy and improve patient compliance.Minimize adverse drug reactions.In recent years,there has been an increase in concentration monitoring of antidepressant medication.Duloxetine is SNRIs drugs,whose study between the blood drug concentration and antidepressant treatment is less,relations are not clear,and the study of the relationship between different target symptoms basic is blank.This study will explore the relationship between the treatment effect and the core symptoms and somatic symptoms in patients with depression between different concentrations of Duloxetine.Methods: Choose 70 patients diagnosed with depression at our college mental health clinic of in March 2017 to October 2017.They were randomly divided into two groups A group of 80 mg group and B group 60 mg,respectively taking fixed dose of duloxetine hydrochloride enter ic capsule treatment of 6 weeks.At baseline and after treatment 1weekend,2weekend,6weekend for determination of HAMD scores HAMD core factor and HAMD body factor to assess the clinical efficacy,TESS scale for adverse reactions.HPLC was used to determine the concentration of duloxetine in the first weekend and the second weekend after the treatment.Statistical analysis,the clinical curative effect by FAS set data analysis.The relationship between Duloxetine blood drug concentration and clinical curative effect also uses FAS set data analysis,using T test and Chisquare test,Correlation analysis for statistical analysis.Results:1,Two groups of patients after treatment at different time points(1,2,6)weekend HAMD scale score compared with the baseline had very signif icant statistical significance(P<0.05).A group of 80 mg group compared with B group of 60 mg,in the first weekend and the 2th weekend,the HAMD total score of the A group below The B group,with significant difference(P <0.05),but in 6 weekend,two groups of total score had no significant difference(P>0.05).2,about the core symptoms,the core of A and B groups of factor score in the score of the three points were also significantly reduce the baseline,and in the first weekend,The 2th Weekend,A group’s core factor significantly less in the B group,with significant difference(P<0.05).At the end of the sixth week,the differences between the two groups were not signif icant.3.Somatic symptoms showed no signif icant change in the two groups at the first weekend,and there was no statistical difference between the two groups in the first weekend(P<0.05).All groups began to decrease at the end of the second week,and the 80 mg group was lower than 60 mg group(P<0.05)in the latter 2 weeks.4.The efficiency of the 80 mg group was higher than that of the 60 mg group at the beginning of the weekend,and the effective rate of treatment in the weekend was higher than that in the 60 mg group.There was no significant difference in the clinical cure rate between the two groups over the weekend.The concentration of blood drug in the 80 mg group was greater than 60 mg in the two measurements,and the blood concentration of each group was not different in the same dose(P>0.05).6,The first weekend,the blood drug concentration of patients with more effect comparing to the patient with no effect was higher(P<0.05),The 2th weekend,effective people also had high blood drug concentration.The 6th weekend,blood drug concentration of patients with clinical cure was high,the difference was statistic ally signif icant(P<0.05).7.The blood drug concentration in the 80 mg group was significantly related to HAMD score and core factor score.There was no statistical significance in relation to somatic factor score(P<0.05).There was no significant difference in the serum concentration of 60 mg group with the total score of HAMD,core factor score and somatic factor score.8.The 80 mg group blood drug concentration had signif icant correlationb with the HAMD total score points rate,core factor score rate in 1,2,6 weekend(P<0.05),and body factor score points rate in the first two weeks had obvious correlation.The blood drug concentration of 60 mg group was signif icantly correlated with the total score of HAMD and the score of core factor in the weekend of 1,2 and 6,which was not related to the score of the somatic factor score at 3 points.9.Using ROC curve to find the lowest concentration treatment,according to the rate of HAMD deducted.A group of 80 mg group in the treatment of 2 weekend the effective blood drug concentration,6 weekend clinical cure the blood concentrations of ROC curve meaningful(Az > 0.7,P<0.05).It is concluded that in the use of 80 mg doses,minimum concentration treatment was 46.1ng/ml,clinical cure the lowest concentration of 49.5 ng/ml.However,the minimum concentration of core factor therapy was 46.1ng/ml,with signif icant statistical signif icance(Az>0.7,P<0.05),while the minimum concentration of somatic symptom group treatment was 45.4ng/ml,but the judgment value was low(Az<0.7,P<0.05).There was no signif icant relationship between serum concentration and side effect(P<0.05)in either 80 mg or 60 mg group.There was no statistical difference between the two doses.Conclusion: 1.The treatment of Depression with different doses of Duloxetine was effective;2.At different time points,there were significant differences between HAMD scores,core factors and somatic factor scores.3.There was a significant correlation between the blood drug concentration and the use dose,the total score reduction rate and the core factor reduction rate of the drug.There is a time effect on the relationship between the reduction rate of somatic factors;At the same dose,there were low concentrations of individual blood drugs and poor therapeutic effect.
