Objective To investigate the incidence and severity of dental caries in children with cerebral palsy(CP)and there relationship with Gross Motor Function classification system(GMFCS).Methods From June 2015 to October 2016,100 cases of cerebral palsy with clear treatment,diagnosis and classification were selected(aged 24 ~ 77 months)in Weihai Municipal Hospital Rehabilitation Department.The severity of cerebral palsy was measured by GMFCS,divided them into I~V levels,levels I~II were mild cerebral palsy,level Ⅲ was moderate cerebral palsy,levels IV~V were severe case of cerebral palsy;The Gesell development diagnosis scale(GDDS)was used to assess the intellectual development of all the cerebral palsy children.Dentists diagnosed caries in all children to determine the severity of their caries and calculate the decayed missing filled deciduous teeth(dmft).To detemine the situation with cares on different sexes and types of cerebral palsy,and the low intelligence level.Their status of dental caries were assessed by the same dentist using the calculated dmft and were compared with 50 normal children matched for age and sex.To analyze the prevalence and severity of dental caries in children with cerebral palsy and there relationship with GMFCS grading.Results Incidence of caries among different sex cerebral palsy children was 80.88% in male and 81.25% in female,the difference was no significance(P> 0.05);children with cerebral palsy were divided into six types,which were spastic hemiplegia,spastic diplegia,spastic quadriplegia,dyskinetic,ataxia and mixed types,incidence of caries among different types did not have significant differences(P> 0.05);After diagnosing with Gesell intelligent development scale,incidence of caries among cerebral palsy children with low intelligent development level(DQ<75)was 88.89%,significantly higher than that among cerebral palsy children without low intelligent level(DQ≧75),which was60.71%,the difference was significant(P <0.01);The rate of caries in children with cerebral palsy was 81% and the dmft index was 3.38 ± 2.514.Both were significantly higher than normal control group(caries rate 50%,dmft index 1.32±1.584,P <0.01).There was a positive correlation between dmft and GMFCS in children with cerebral palsy(r =0.393,P <0.01).The more severe the degree of gross motor dysfunction,the more serious the degree of dental caries.Conclusion Caries is a common oral disease of children with cerebral palsy,especially the cerebral palsy children with low intelligent level(the diagnostic score of Gesell’s was DQ <75)and children with severe cerebral palsy(GMFCS levels IV~V).Dental health care should be paid attention to by parents and doctors and it should be incorporated into routine clinical management. |
