Objective:Comparing the advantages and disadvantages of the two perfusion methods in percutaneous nephrolithotomy.Exploring the impact of different perfusion methods on surgery,provide the basis for the clinical choice of appropriate perfusion.Methods:A total of 120 patients who were diagnosed as upper urinary tract stones in Urology Department of Jiangxi Provincial People's Hospital between October 2016and October 2017 and treated with percutaneous nephrolithotomy.According to different intraoperative perfusion methods,they were randomly divided into A group(Endoscopic injection pump)and B group(Gravity perfusion method).The following factors were recorded:the average pelvic pressure,the maximum pelvic pressure,postoperative fever,time of perfusion,volume of perfusion,residual stones,procalcitonin content,the rate of postoperative blood leukocyte counting(>10*l0~9/L).Results:All the patients in the two groups completed the operation successfully.A total of 14 cases of postoperative fever occurred,accounting for 11.7%.Both groups of patients have not died or septic shock..There was no significant difference between the A group and the B group in postoperative fever(P=0.34).The average pelvic pressure of the A group 24.63±3.77 mmHg,and the B group:18.25±2.48 mmHg.There was a significant difference between the A group and the B group in the average pelvic pressure(P=0.026).The statistics was significantly different in the maximum pelvic perssure of the A group and the B group.There was no significantly diference between the two groups in the rate of residua l stones(P=0.778),time of perfusion(P=0.269).There was a significant difference between the A group and the B group in procalcitonin content(>0.1ng/L),volume of perfusion and the rate of postoperative blood leukocyte counting(>10*l0~9/L).Conclusions:1.There were no significant differences in postoperative fever in percutaneous nephrolithotomy between the two different perfusion methods,but the potential risk of postoperative infection in the natural perfusion group was lower than in the infusion pump group.2.There was no significant difference in stone removal efficiency between the two perfusion methods.3.There were no significant differences in the time of surgery between the two perfusion methods.
