Objective:To explore the practical value of high frequency color Doppler ultrasonography in the diagnosis of breast ductal carcinoma in situ and to analyze the correlation between the ultrasonic manifestations of the lesions and the pathological grades,so that we can provide reliable ultrasonic diagnosis basises to make a reasonable,comprehensive and individualized therapy diagnosis for the patients with DCIS.Methods:The ultrasonographic features of 132 cases of breast DCIS confirmed by operation and pathology in our hospital were studied retrospectively.According to whether there are malignant calcification in mammography,they can be divided into two groups: no malignant calcification and malignant calcification.Ultrasonic examination results can be divided into normal ultrasonography,mass type,simple dilatation catheter type,catheter dilatation with formation of endoplasmic mass,and layered uneven/hypoechoic area.Thus,the correlation between DCIS and sonographic findings at different pathological grades were studied.Results:Among 132 cases of breast ductal carcinoma in situ,69 cases(52.3%)were malignant calcification group and 63 cases(47.7%)were nonmalignant calcification group.Respectively divided into two groups of breast sonographic findings were:mass type 31cases(23.5%)and 37cases(28.0%),simple conduit dilated,5 cases(3.8%),and 2 cases(1.5%),tube expanding with its mass formed in 5 cases(3.7%)and 7 cases(5.3%),flake heterogeneity/keyed acoustic area 20 cases(15.2%)and 12 cases(9.1%),negative 8cases(6.1%)and 5 cases(3.8%).Ultrasound in shape,border,internal echo and blood flow situation related to the pathological grading(r=-0.301?-0.283?-0.286?-0.284,P<0.005),high grade associated with tumor type,lamellar heterogeneous in malignant calcification group/ hypoechoic zone type(r=0.287,0.271,P< 0.005),middle grade associated with ductal dilatation and mass formation in malignant calcification group(r=0.309,P<0.05),low grade associated with tumor type in nonmalignant calcification group and negative ultrasonic findings(r==0.2650.271,P<0.05).Conclusion: There is a correlation between DCIS ultrasonic images and pathological grades.Ultrasonic examination is of great significance in optimizing the treatment of DCIS which the disease with invasive development potential.
