Meta-analysis On Clinical Characteristics Of Recurrent Low-frequency Hearing Loss

Objective: To systematically evaluate the clinical characteristics of recurrent low-frequency sensorineural hearing loss(RLFSHL)by meta-analysis,analyze the relationship such as morbidity,influencing factors and differential pionts,in order to providing the references of clinical diagnosis and therapy of related diseases.Methods: 1.The Cochrane Library and PubMed,Chinese biomedical literature database,and Chinese journal of biomedical literature database,and China National Knowledge Infrastructure(CNKI),and VIP of Chongqing,and Chinese medical academic conference papers database(CMAC),and Wanfang databases were searched to collect all the papers about low-frequency sensorineural hearing loss until December 2017.Statistical analysis was performed using Cochrane and Stata 15 software.The quality of the literature was evaluate by the observational quality assessment tolls provided by the Cochrane,and grading strictly by the inclusion criteria and exclusion criteria.To analyze the morbidity,and influencing factors,and clinical characteristics and differential points of the RLFSHL by Revman5.3 and Stata15.2.The same patients from the Suining central hospital otolaryngology head and neck surgery and the First Affiliated Hospital of Chongqing Medical University otolaryngology head and neck surgery during the last 6 years.Results: A total of 54364 cases including 2736 cases patients with RLFSHL in fifteen studies(from 2002 to 2016)were included.The recurrence rate of RLFSHL is1.0%.The exposure rate of Meniere disease is 1.6%(44/2736),and the exposure rate of acoustic neuroma is 0.07%(2/2736).26 patients with RLFSHL screened out from those two hospitals' patients with sudden deafness.All the 26 patients were single ear disease,1 case recrudesced with vertigo,and 3 cases companied with migraine and photophobia.For the first time patients suffered from RLFSHL,16 cases were cured,4 cases were effective,and 6 cases were invalid.On the other hand,for the recurrent patients,14 patients were cured,1 patient was effective,12 patients were ineffective,and 3 cases were aggravated.The accompanying symptoms were tinnitus,ear stuffy,dizziness,walking instability and so on.No one patient were diagnosed as Meniere's disease,Migraine,Autoimmune inner ear disease,Temperature sensitive acoustic neuropathy,Acoustic neuroma,or Vestibular paroxysmia.In 26 cases,the morbidity of young women is 77%(20/26),the first onset age was 17-48 years old,and an average age was 31.15±8.16 years old.The recurrent age is 17-51 years old,and an average age was 32.15±8.78 years old.Patient's first time to visit hospital was 1 day to 1.5 months,the average was 10 days.For the recurrent patients,the pause was 8days to 5.5 years.For the first treatment patients,the complete recovery rate was61.54%(16/26),the effective rate was 15.38%(4/26),and the ineffective rate was23.08%(6/26).For the retreatment patients,the complete recovery rate was 53.85%(14/26),the effective rate was 3.85%(1/26),the ineffective rate was 34.62%(9/26),and the worsening rate was 1.54%(3/26).The positive rate of inner eardisease was low,but with the maturity of audiological examination in recent years,patients with RLFSHL were diagnosed as final disease gradually improved.Conclusions: 1.The incidence of RLFSHL is relatively low,which is often misdiagnosed as sudden deafness,and its cure rate is low.2.The detection rate of patients gradually increased,and very few were eventually diagnosed as related diseases such as Meniere's disease.3.Patients with RLFSHL need to be followed up for a long time.The pathophysiological process needs more scholars to continue to explore and study.
