Background:Acute low-frequency sensorineural hearing loss was first reported by Abe in 1982.Patients with acute onset, dominated by low-frequency hearing loss, high-frequency hearing is normal. Most of the degree of hearing loss is not heavy. Hearing loss gradually reduces and restores to normal with the increase of frequency. After a clinical summary and study, now inclined to think that is a separate disease. Because the cause of disease is unknown, and at present there is no uniform treatment. Corticosteroids, vasodilators agents and neurotrophic agent are often used for the treatment of the disease and make a good prognosis.Objective: Acute low-frequency sensorineural hearing loss in clinical features, diagnosis, treatment and effects are discussed to enhance the awareness of clinicians and treatment of the disease, reduce misdiagnosis ,missed diagnosis and the pain of patients , ease the financial burden.Methods: 40 patients (41 ears) diagnosed as acute low-frequency sensorineural hearing loss were treated in outpatient or inpatient of the First Affiliated Hospital of Dalian Medical University, Department of Otorhinolaryngology from April of 2009 to October of 2010. High-dose prednisone was taken by them orally every morning. (1 mg/L·d,1-3d;0.5mg/L·d,4d;0.25mg/L·d,5d;5mg/d,6d) .At the same time, vasodilating medicines and protecting nerves drug were taken,and 7·day is an treatment course.All patients had the otological examination and pured tone audiometry to observe the recruitment of hearing pre-therapy and after 7-day treatment at all-frequencies(125,250,500,1 000,2000,4000,8000Hz).We analyzed all the datas with SPSS11.5 using t test or chi-square test . The result of 40 cases (41 ears) with acute low-frequency sensorineural hearing loss was retrospective analyzed in clinical features, audiological test results, treatment and 3months to 22 months of follow-up situation.Clinical characteristics and treatment of conditions of ALHL were Summary.Results: The 40 cases (41 ears) with acute low-frequency sensorineural hearing loss showed acute onset patients, aged 17 to 51 years, mean (31.48±7.78) years. 13 males (14 ears) (32.50%), female 27 cases (27 ears) (67.50%). Of these, 29 cases (29 ears) with a low-key tinnitus, 26 patients (27 ears) with ear swelling and blocked a sense of nausea, 18 patients (18 ears) What is hearing loss, 9 patients (9 ears), dizziness associated with influenza, and 6 cases (6 ears) with auditory allergy, 3 patients (3 ears) with enhanced self-listening. After treatment, 33 patients (34 ears) complete recovery, 5 patients (5 ears) partial recovery, 2 patients (2 ears) no change. The recovery of hearing occurred within 5 days after treatment .Before treatment an average of 0.125 ~ 0.500 kHz threshold was (42.03±9.28) dB HL, 2 ~ 8 kHz hearing threshold was (16.59±3.43) dB HL, the average hearing thresholds after treatment were (20.45±8.66) dB HL and (15.81±2.94) dB HL.The effective rate was 95.12%. Cure rate was 82.93%. The effective treatment of women was 96.30%, the cure rate was 85.19%. The effective treatment of man was 92.86%, the cure rate was 78.57%. Treatment of female and male patients has no significant difference in efficiency (P> 0.05).Conclusion : ALHL are more common in young women with unilateral disease-based, often accompanied by a low-key tinnitus, ear fullness and nausea block. Determination of pure tone thresholds performance low-frequency hearing threshold increased as the characteristics. Comprehensive treatment is effective.
