Objective:The purpose of this study is to design and build a cloud platform which can provide transitional care service and management for patients with type 2 diabetes after discharge by constructing the index system of functional modules of the“Comprehensive information service cloud platform of transitional care for diabetes”based on WeChat enterprise.It will help diabetics gain more interactive,convenient,accessible,intelligent and individualized transitional care service,improve patients' self-management ability and quality of life.Methods:Using methods of literature analysis,software function collection and brainstorming,we draw up the index system of functional modules of the cloud platform.Twenty four experts from the related fields were consulted by Delphi method on the importance and feasibility of the index system.Excel2003 and SPSS17.0 were used to analyze the average score,standard deviation,coefficient of variation and Kendall coordination coefficient of two rounds of indexes.Our research team discussed and decided to increase or decrease the two rounds of indicators according to the criteria of index selection.The frequency and constituent ratio are used to describe the basic situation of the experts,and using positive coefficient and authority coefficient to determine the enthusiasm and authority of the experts,respectively.Using Kendall coordination coefficient and coefficient of variation reflects the coordination degree of expert opinion.And the above indexes are used to test the reliability of the consultation results.According to the index system of function modules,Our research team will assist software engineers in designing“Comprehensive information service cloud platform of transitional care for diabetes”by means of object relation mapping and rapid development platform.Then we deploy it to WeChat enterprise.Results:(1)The index system of “Comprehensive information service cloud platform oftransitional care for diabetes” was constructed,including 13 first-level indexes,50second-level indexes and 143 third-level indexes.The positive coefficients of two rounds of experts were 87.50% and 90.48%(>70%),respectively and the authority coefficient of two were 0.84(>0.7).The Kendall coordination coefficient of importance of two rounds of the third level indexes were 0.540 and 0.597(P<0.01),respectively.The Kendall coordination coefficient of feasibility were 0.359 and 0.563(P<0.01).The coefficient of variation of importance and feasibility of each index were 0.05~0.35 after the second round of expert consultation.(2)The “Comprehensive information service cloud platform of transitional care for diabetes” has been initially developed,including 13 functional modules of patient information,medical information,health assessment,daily record,health consultation,health guidance,follow-up management,health information,patient self-rating,friend topic,intelligent reminding,relatives management and administrator management.And the specific functions are being further built.Conclusions:(1)The index system of “Comprehensive information service cloud platform of transitional care for diabetes” has been constructed with 13 functional modules,such as “patient information”,“medical guidance” and “intelligent reminding”.The index system included 13 first-level indexes,50 second-level indexes and 143 third-level indexes.The results had high reliability tested by positive coefficient,authority coefficient and Kendall coordination coefficient.(2)We have initially designed and developed the “Comprehensive information service cloud platform of transitional care for diabetes”based on WeChat enterprise.And the specific functions are being further built.The practical effect of application needs to to be further studied and evaluated.
