Objective:To investigate preliminarily the changes of bone marrow fat fraction(FFmDIXON,FFMRS)and T2*value of lumbar vertebrae measured by mDIXON and 1H MRS in different bone mineral density?BMD?groups, their correlations with BMD and diagnostic applications in the postmenopausal osteoporosis. Methods:To collect 60 pa-tients who underwent dual energy X-ray absorptiometry?DXA?,routine magnetic reso-nance imaging?MRI?and mDIXON,1H MRS in the lumbar vertebrae and measure the bone mineral density?BMD?,T-score,FFmDIXON,FFMRS,and T2*values of the lumbar vertebral 3?L3?.According to the T-score,the patients were divided into the normal,os-teopenia and osteoporosis groups. To analyze the differences of FFmDIXON,FFMRS and T2*values among the 3 groups used one-way ANOVA,the correlations with BMD by the Pearson correlation, the diagnostic capabilities in osteoporosis by the receiver operating characteristic curves?ROC curve?,the concordance of FFmDIXON and FFMRS used the in-tra-class correlation coefficient?ICC?.Results:The differences of FFmDIXON,T2*values,FFMRS in the 3 groups were statistically significant?P<0.05?;the correlation coefficients with BMD were respectively r1=-0.609,r2=-0.573,r3=-0.530?P<0.05?;the areas under the ROC curves?AUC?respectively were 0.894±0.046,0.761±0.068,0.873±0.049;the intra-class correlation coefficient between FFmDIXON and FFMRS was 0.855?P<0.05?.Conclusion:mDIXON is a robust and simple method for accurate quantification of fat content and is highly consistent with 1H MRS,can accuratly quantify fat content could evaluate the lumbar vertebral bone marrow fat content, reflect the changes of the bone quality, provide valuable information for the diagnosis of osteoporosis, and is helpful to predict the risks of the lumbar fragility fracture, may have broad application prospects in both research and clinical practice.
