Clinical Observation Of Syndrome Elements,constitution And Primary Collaterals In Patients With Carotid Artery Stenosis

ObjectiveTo investigatethe characteristics of constitution and TCM syndrome elements of carotid artery stenosis(above 50%degree),and the relationship between constitution,syndrome elements and primary collaterals.MethodThis is a cross-sectional study.The patients were diagnosed with carotid artery stenosis(?50%)by vascular ultrasonography or MRA,symptomatic group(cerebral infarction or TIA)and asymptomatic group were contained.Some patients underwent cranial or cervical CTA examination to record the location,degree of stenosis,and the circle of Willis.The general data,syndrome elements,physical constitution and NIHSS scale were used to evaluate neurological impairment.Symptoms elements and NIHSS scores were recorded according to onset first,third,seventh,14th and 28th day in the symptomatic group.The compensatory mode of Willis circle in patientswith severe carotid artery stenosis and occlusion were analyzed.Results1.Constitution distribution of patients with internal carotid artery stenosisThe top 3 of the patients with carotid artery stenosis are phlegm dampness(25.8%),ideal constitution(21.0%),blood stasis(12.9%),Yang deficiency(11.3%),Yin deficiency(11.3%),Qi Stagnation(7.9%)and there is no Special constitution.2.Characteristics of syndrome elements in patients with internal carotid artery stenosis?Evolution of single factor of symptom syndrome:The wind permits the maximum of first days,has since been dropped.The proportion of blood stasis syndrome and phlegm dampness is higher and remain unchanged.The qi deficiency,internal heat syndrome increased gradually since the first day.and yin deficiency syndrome uncharged.?Proportion of syndrome factors:Combined analysis of symptom symptoms in 28th days in symptomatic group and asymptomatic group.The percentage of patients with no symptoms followed by blood stasis,Qi deficiency,fire,phlegm3.Wiilis compensation and prognosis in symptomatic severe carotid artery stenosis/occlusionIn patients with symptomatic severe carotid artery stenosis/occlusion,the proportion of ACoA is higher than that of the patients with PCoA and the both.Compared with the ACoA open group,there was no difference in NIHSS score,BI score and mRS score between the 2 groups.However,compared with the 90th day NIHSS,BI score and mRS score,there was significant difference in the ACoA open group(P<0.05).The BI index was statistically significant in the unopen group(P<0.05).4.Relationship between syndrome elements,constitution and Wllis circle In the patients with symptomatic severe carotid stenosis,the proportion of internal fire syndrome,blood stasis syndrome,Qi deficiency syndrome and yin deficiency syndrome were higher than those in the ACoA open group,and the combination of syndrome factors is more complex.In terms of physical distribution,there was no difference between the two groups.
