BackgroundDepression after ischemic stroke is the most common neuropsychiatric complication after stroke. This disease has a significant effect on the cognitive function of patients and the recovery and prognosis of primary diseases. Its diagnosis is rather complicated and is based on some clinical parameters. In recent years, people have sought to find biomarkers associated with ischemic post stroke depression in some large databases, but there is no evidence to support the use of specific biomarkers for ischemic post stroke depression. However, many studies have shown that immunomodulatory disorders play an important role in the pathophysiological process of depression. Therefore, immunological studies in the field of Th cytokines may provide a personalized method for stroke patients, and also help to diagnose and treat such common neuropsychiatric disorders.ObjectiveThe expression level and clinical significance of Th cytokines in patients with post-stroke depression were investigated by flow cytometry.Methods80 first-episode ischemic stroke patients who were hospitalized in First Affiliated Hospital of Xinxiang Medical College neurology from February 2016 to October 2016 were selected to complete routine imaging examination and give general medical treatment.Follow up to the ninetieth day early morning, use anticoagulant tube to extract 3ml from the empty belly of the elbow, put it in the refrigerator at-80 centigrade, and use Jian Yizhi. The mental state examination score(MMSE),the Montreal cognitive score(MoCA)assessed the patient's cognitive function, assessed the patient's daily living ability with BI(daily living ability),and assessed the patient's mental state with the HAMD scale.DSM-V depression diagnostic criteria and HAMD-17 item scale were used to score and group depression. The Hamd score of 730 cases in the depressive group and 33 cases in the non-depressive group(17 cases who dropped off during the experiment)were compared with the general clinical data(including age, sex, smoking, alcohol consumption). The difference between cognitive and functional scores associated with the nervous system. In addition,30 healthy people in the same period were selected as control group. The expression of Th cytokines in peripheral blood of the above three groups was detected by flow cytometry.Result1.There was no significant difference in clinical data and neurological scores between depression group and non depression group(P > 0.05).2.The level of IL-2 in the depression group was significantly lower than that in the non depression group(P < 0.05),and the level of IL-2 in the two groups was significantly lower than that in the healthy group(P < 0.05).The levels of IL-10,IL-17 a and IFNgamma in the depression group were higher than those in the non depression group(P < 0.05),and the average of IL-10,IL-17 a and IFN-gamma water in the two groups was higher than that in the healthy group(P < 0.05).ConclusionsThere is no significant relationship between depression after ischemic stroke and the general data of the patients and the score of the nervous system. It is concluded that there is a certain degree of immune imbalance in the patients with depression after stroke and the abnormal expression in the patients with depression after stroke. The expression of low expression of Th cytokine IL-2 and high expression of IL-10,IL-17 a and IFN-gamma showed that apoplexy and post stroke depression could lead to abnormal immune inflammatory response. This study will be expected to provide a basis for the prevention and diagnosis of post-stroke depression.
