Objective: The purpose of this study was to explore the value of transvaginal two-dimensional doppler ultrasound combined with real-time three-dimensional imaging in the diagnosis of intrauterine occupying lesions(endometrial polyps,uterine myoma and endometrial cancer)and the preoperative staging of endometrial carcinoma.Methods: The clinical data of 184 cases of intrauterine space occupying lesions admitted to our hospital from April 2015 to April 2017 were retrospectively analyzed.All patients underwent transvaginal two-dimensional doppler ultrasound and real-time three-dimensional reconstruction.The results were recorded,hysteroscopy or gynecologic surgery were performed,and biopsy or surgical specimens were examined histopathologically.The difference between the transvaginal two dimensional doppler ultrasound combined with the real-time three-dimensional imaging technique and the transvaginal two-dimensional doppler ultrasound in the detection rate of pathological changes,the postoperative pathologic coincidence rate and the accuracy rate of preoperative staging were compared with the pathological examination results as gold standard.Results: 1.50 cases were diagnosed with endometrial polyps by pathology.In transvaginal two dimensional doppler ultrasound combined with the real-time three-dimensional imaging technique group,the detection rate(42 cases,84% vs 33 cases,66%)and postoperative pathological coincidence rate(42 cases,84% vs 33 cases,66%)by were significantly higher(P <0.05).2.100 cases were diagnosed with uterine fibroids by pathology.In transvaginal two dimensional doppler ultrasound combined with the real-time three-dimensional imaging technique group,the detection rate(88 cases,88% vs 70 cases,70%)and postoperative pathological coincidence rate(88 cases,88% vs 70 cases,70%)by were significantly higher(P <0.05).3.34 cases were diagnosed with endometrial carcinoma by pathology.In transvaginal two dimensional doppler ultrasound combined with the real-time three-dimensional imaging technique group,the detection rate(30 cases,88.24% vs 23 cases,67.65%)and postoperative pathological coincidence rate(29 cases,85.24% vs 24 cases,70.59%)by were significantly higher(P <0.05).4.The accuracy rate of preoperative clinical staging of endometrial carcinoma by transvaginal two dimensional doppler ultrasound combined with real-time three-dimensional imaging(29 cases,85.29%)was significantly higher than that of transvaginal two-dimensional doppler ultrasound(24 cases,70.59%),(P <0.05).Conclusion: For patients with endometrial polyp,uterine fibroids and endometrial cancer,routine transvaginal two-dimensional doppler ultrasonography was performed and three-dimensional ultrasonography was used to rebuild pathological changes to shape three-dimensional structure dynamically,which was able to show related structures' three-dimensional shapes,interior structure in the color flow imaging,to make a more accurate diagnosis and differential diagnosis of uterine occupied lesions than transvaginal two-dimensional doppler ultrasonography and to perform more correctly the preoperative staging ofendometrial carcinoma compared with transvaginal two-dimensional doppler ultrasonography.
