Objective:The objective is to observe the clinical efficacy on the tr-eatment of heel pain mainly with triple needling.The relations-hip between the courses and the treatment based on syndrome di-fferentiation were compared with score index.It also provides a reference for the selection of clinical acupuncture method,th-e control of treatment course and the treatment based on syndr-ome differentiation.Methods:31 cases(male 14,female 17)patients had been collected from the outpatients and inpatients from November 2015 to December 2017 in the department of rheumatology and acupuncture in Wuhan hospital of traditional Chinese medicine,who corresponding to the diagnosis,inclusion and exclusion criteria of heel pain.There were 20 people in the type of liver-kidney asthenia and 10 people in the type of obstruction by blood stasis.They all were treated by using ashi point with triple needling(depth about 1.0-1.2 inches).Ashi points were stimulated with acusector.Besides,taixi and sanyinjiao were stimulated with acusector in the type of liver-kidney asthenia.Cheng shan was stimulated with acusector in the type of obstruction by blood stasis.The observation period was 2 weeks.In the treatment process,1 case was lost(due to intolerance and the fear of acupuncture in the heart,only having 1 times of treatment,then changing to external drugs),and the rest 30 cases(14 males and 16 females)were included in the statistics.All the cases were compared with themselves.3 times treatments per week(one times every other day),3 times for one course of treatment,2 courses of total treatment,2 days of rest after one course of treatment.Efficacy was evaluated after 1 course and 2 courses of treatment.The clinical curative effect(see "standard of diagnosis of disease and curative effect of traditional Chinese medicine"),simplified McGi11 rating scale(including pain rating index PRI,visual analogue scale VAS,present pain intensity PPI)and discomfort records of patients are included.SPSS17.0 statistical software was used for processing,and the rank sum test was used to compare the therapeutic indexes.P<0.05 indicates that the difference detected by statistical results is statistically significant.Results:(1).Results of overall clinical efficacyIn the 30 cases(30 feet)patients,10 cases were cured(33.3%),6 cases were markedly effective(20.0%),9 cases were effective(30.0%),5 cases were invalid(16.7%)after one course of treatment,while 16 cases were cured(53.3%),8 cases were markedly effective(26.7%),5 cases were effective(16.7%),1 case was invalid(3.33%)after 2 courses of treatment.The respective total efficiency is 83.3% and 96.7%.After one course of treatment,17 cases(85.0%)are effective and 3cases(15.0%)are ineffective in the type of liver-kidney deficiency.8 cases(80.0%)are effective and 2cases(20.0%)are ineffective in the type of obstruction by blood stasis.After 2 courses of treatment,19 cases(95.0%)are effective and 1 case(5.0%)is ineffective in the type of liver-kidney deficiency.10 cases(100.0%)are effective and no case(0.0%)is ineffective in the type of obstruction by blood stasis.(2).The results of the Simplified McGi11 scale and the type comparisonThe patient's PRI feel points and PRI mood points were compared after one course of treatment and 2 courses of treatment,and after one course of treatment,the degree of remission was found to be more than 2 courses of treatment.Besides,the scores were further reduced after 2 courses of treatment.VAS and PPI scores were compared before and after the treatment,indicating a significant decrease in VAS and PPI scores after the treatment.And P<0.05,the differences in the scores were statistically significant.The therapeutic effect was compared between the patients with liver-kidney deficiency and blood stasis block after 1 course and 2 courses of treatment.And P > 0.05,the difference in efficacy was not statistically significant.Conclusion:Taking ashi points and using acusector are point to dredge blocked channels,relax the muscles and tendons and relieve the pain.Combined with the selection of the affected side Taixi,nourishing the kidney-Yin.Chengshan,promoting qi to activate blood.Sanyinjiao can can be used to improve the liver and kidney.The treatment is effective.It can obtain obvious curative effect and effectively improve clinical symptoms in patients with heel pain.It can also intervene in the patient's emotional state,reduce patient's pain,promote clinical recovery and finally improve the patient's quality of life.Through the clinical meta-analysis,it was found that conservative treatment had different effects on the length of treatment and the long-term efficacy.Through this clinical observation,we found that triple needling is safe,effective,simple to operate.The curative effect of heel pain is obvious and rapid.Triple needling is worth popularizing in the heel pain treatment.Moreover,after one course of treatment and 2 courses of treatment,the therapeutic are all effective.The subjective pain relief degree is more significant after one course of treatment.However,in general,the effective rate was significantly improved after 2 courses of treatment comparing with 1 course of treatment,and the cure rate and marked improvement rate were also increased,which was more suitable as a reference for the treatment course.In addition,there is no obvious difference in the therapeutic effect between the two types,which may be the reason that the sample size is too small.
