| Objective:To search for a new treatment and effective method for myofascial pain by acupuncture stimulation to Ah Shi points. The object of observation was clinically common myofascial pain syndrome. The research contains three parts, the first part is a study of theory and document, the second part is the clinical study of new acupuncture method of Ah Shi points for myofascial pain, the third part is the controlled study of new acupuncture method of Ah Shi points and routine treatment for myofascial pain.Mehods:60(or90) patients who were suffering from myofascial pain at the same place were divided into two(or three) groups randomly. Each experiment included the treatment group(s) and control group. Each group had thirty cases.The diagnosis criteria of myofascial pain are referred pain and local twitch response. To ensure the treatment group(s) and the control group were comparable, ratios of male to female, ages, courses of disease, and pain scales before acupuncture were assessed. The direction of horizontal pressure to trigger point(taut-band) which patient felt most painful was defined as the pain direction. Handy Push-Pull Gauge was used to measure the pressure power on Ah Sih points in each experiment. The unified manipulations included pressure site, direction and power were set. The depth of needling is that the top of the needle reaches the trigger point(taut-band). All of these clinical experiments used Numerical Scale of Visual Analogue Scale(VAS). Evaluation criterion of curative effect:average pain scale reducing rate,complete pain reducing rate, apparent pain reducing rate, partial pain reducing rate, absence of pain reducing rate, obvious pain reducing rate, and total effective rate. Statistic analysis:The following were used:value approach probability, paired samples test and two-tailed test.Results:The clinical study of new acupuncture method of Ah Shi points for myofascial pain:1.The experiment of needling direction:This is a study on the effect of therapy of back myofascial pain syndrome by different needling direction on Ah Sih points.The analgesic effect of distant acupuncture has very significant difference (P< 0.01). The analgesic effect of needling towards the tender spot and across it has no significant difference (P> 0.05). The average pain scale reducing rate, obvious pain reducing rate and total effective rate of distant acupuncture are much better than those of the latter. Statistically,there are very significant differences in pain relief between distant acupuncture and the other two methods (P<0.01). There is significant difference in pain relief between needling towards the tender spot and across it (P< 0.05). Among them the distant acupuncture is most effective.2.The experiment of needling angle:This is a study on the effect of therapy of back myofascial pain syndrome by different needling angle on Ah Sih points. The analgesic effect of horizontal needling (15°) has very significant differencs(P< 0.01). The analgesic effect of oblique needling(45°)has significant difference (P< 0.05).The analgesic effect of vertical needling (90°) has no significant difference (P>0.05). The average pain scale reducing rate, complete pain reducing rate and obvious pain reducing rate of the horizontal needling are higher than those of the other two methods. The total effective rates ofthe horizontal and oblique needling are much higher than that of the vertical needling. Statistically, there are very significant differences in pain relief between horizontal needling and the other two methods (P<0.01). There is significant difference in pain relief between oblique needling and vertical needling (P< 0.05). Among them the horizontal needling is most effective.3.The experiment of needling density:This is a study on the effect of therapy of upper arm myofascial pain syndrome by different needling density on Ah Sih points. The analgesic effect of low needling density (1 needle/1cm),moderate needling density (3needles/1cm) and high needling density (5 needles/1cm)have very significant differences (P<0.01). The total effective rates of all the three acupuncture density are 100%. The more acupuncture density, the more obvious pain reducing rate. The obvious pain reducing rates of the second and third are much higher than that of the first. Statistically, there are not significant difference of analgesic effect between the three methods (P> 0.05)4. The experiment of needling time:This is a study on the effect of therapy of back myofascial pain syndrome by diffierent needling time on Ah Sih points. The analgesic effects of needling 5 minutes,15 minutes and 25 minutes have significant diffierences (P<0.05). The total effective rates of needling 5 minutes,15 minutes and 25 minutes are all 100%. The longer the needling time, only the little average pain scale reducing rate increase. There are not significant differences of analgesic effect between the three methods (P> 0.05). If the needling method is appropriate, the analgesic effect is satisfying to needle only for 5 minutes. If the needling time increased, the further analgesic effect improvement is finite.The controlled study of new acupuncture method of Ah Shi points and routine treatment for myofascial pain.1.The curative effect comparison of new acupuncture method of Ah Shi points and local-distant points selection for lateral elbow myofascial pain syndrome:This is a study on the effect of therapy of lateral elbow myofascial pain syndrome by new acupuncture method of Ah Shi points and local-distant points selection. The analgesic effect of new acupuncture method of Ah Shi points has very significant diffierence (P<0.01). The analgesic effect of local-distant points selection has not significant diffierence (P>0.05).There is very significant difference of analgesic effect between the two methods (P< 0.01). 2. The curative effect comparison of new acupuncture method of Ah Shi points and wrist-ankle acupuncture for back myofascial pain syndrome:This is a study on the effect of therapy of back myofascial pain syndrome by new acupuncture method of Ah Shi points and wrist-ankle acupuncture. The analgesic effect of new acupuncture method of Ah Shi points has very significant diffierence (P<0.01).The analgesic effect of wrist-ankle acupuncture has not significant diffierence (P>0.05). The average pain scale reducing rate, apparent pain reducing rate, obvious pain reducing rate and total effective rate of new acupuncture method of Ah Shi points are much better than those of wrist-ankle acupuncture. There is very significant difference of analgesic effect between the two methods (P<0.01)3.The curative effect comparison of new acupuncture method of Ah Shi points and corresponding acupoints for medial wrist myofascial pain syndrome:This is a study on the effect of therapy of medial wrist myofascial pain syndrome by new acupuncture method of Ah Shi points and corresponding acupoints. The analgesic effect of new acupuncture method of Ah Shi points has significant diffierence (P<0.05). The analgesic effect of corresponding acupoints has not significant diffierence (P>0.05). The average pain scale reducing rate, apparent pain reducing rate, obvious pain reducing rate and total effective rate of new acupuncture method of Ah Shi points are much better than those of corresponding acupoints. There is very significant difference of analgesic effect between the two methods(P< 0.01)Conclusions:This is a study on the new method of myofascial pain by acupuncture stimulation to Ah Sih points. The analgesic effect of distant acupuncture is much better than those of needling towards the tender spot and across it. The analgesic effect of horizontal needling is much better than that of oblique and vertical needling. If the top of needle reaches the Trigger point(taut-band),although the needling density is low (1 needle/1cm), the analgesic effect is satisfactory. The more needling density, the more analgesic effect. But, statistically, there are not significant differences of analgesic effect between three needling densities. If the treatment is appropriate, although the needling time is 5 minutes there are very satisfactory analgesic effect. Although the acupuncture time is prolonged, the increase of analgesic effect is finite. There is very significant difference of analgesic effect between new acupuncture method of Ah Shi points and local-distant points selection. There is very significant difference of analgesic effect between new acupuncture method of Ah Shi points and wrist-ankle acupuncture. There is very significant difference of analgesic effect between new acupuncture method of Ah Shi points and corresponding acupoints. |
