| Ankle osteoarthritis(AOA),also known as ankle osteoarthritis,occurs in middle-aged and old people and is often caused by factors such as degeneration,strain,or injury.The incidence is higher than that of knee and hip.To be less,it belongs to the category of "bone sputum" and "deafness" in traditional medicine.This disease is a chronic aseptic inflammatory joint disease in which the articular cartilage degenerative changes in the ankle joint and various secondary bone hyperplasia are the main pathological changes.The clinical symptoms are mostly ankle joints.Sustained pain,swelling malformations and varying degrees of limited activity.The clinical features such as long duration,complicated conditions and poor prognosis all have a great impact on the patient’s life and psychology.The clinical use of oral drugs and local physical therapy,often only temporarily relieve symptoms,long-term efficacy is poor.In recent years,with the development of medical technology,surgical procedures such as ankle arthroscopy and ankle fusion have emerged,but there are a series of problems such as large trauma,related complications,and high economic costs.How to quickly and effectively reduce the suffering of patients,shorten the course of the disease,and at the same time reduce the economic costs of patients,thus improving the quality of life of patients,has always been a problem to be explored in orthopedic clinical practice.Clinically,on the basis of the knee osteoarthritis(KOA)treatment study,we found that the use of sodium hyaluronate plus triamcinolone acetonide for intra-articular injection of the sacroiliac joint used together with traditional Chinese herbal fumigation to treat AOA can achieve a good curative effect.Objective Make groups and collect relevant data of AOA patients in the orthopedic outpatient department of Wuhan Hospital of Traditional Chinese Medicine,and then conduct statistics and analysis to investigate the clinical efficacy of this combination Chinese and Western medicine treatment of AOA: sodium hyaluronate and triamcinolone acetonide acetate intra-articular injection of ankle joints used together with orthopedic external fumigation and TCM herbal fumigation and washing.Method This study was conducted in two groups:the control group and the treatment group.Then we retrospectively applied statistical analysis to 76 cases of AOA patients in the orthopedic outpatient department of Wuhan Hospital of Traditional Chinese Medicine from December 2015 to December 2017.All patients were collected under the inclusion criteria and acceptance criteria: in the control group,only sodium hyaluronate and triamcinolone acetonide acetate was used for intra-articular injection of ankle joints in 38 patients,including 14 males and 24 females;In the treatment group,sodium hyaluronate and triamcinolone acetonide acetate was used for intra-articular injection of ankle joints in combination with TCM herbal fumigation was performed in 38 patients,including 16 males and 22 females.Patients in both groups were treated for 5 weeks,1 course of treatment,and were followed up for 3 months after the end of the treatment.All clinically relevant data of all patients were collected before treatment,after treatment and for 3 months after the treatment.Visual analogue score(VAS)and Kofoed ankle scores(KAS)were used to evaluate the curative effect,the efficacy evaluation scores were statistically analyzed using SPSS21.0 software.Both groups were followed up comparing before treatment,after treatment,and until 3 months after the treatment using the relevant scores to assess clinical efficacies in the treatment of ankle joint intra-articular injection and TCM herbal fumigation for AOA.Result The VAS score,Kofoed score and clinical cure rate were compared between the two groups of the control group and the treatment group before,after treatment and after the treatment in March.1.The VAS scores of the two groups before treatment were compared,P >0.05,the difference was not statistically significant;both VAS pain scores decreased after treatment and three months after treatment.Both groups were compared with the VAS scores before treatment,P <0.01.The difference was statistically significant,and the difference between the two groups was P <0.01,and the difference was statistically significant.The treatment group was followed up after the treatment and after the treatment was followed up for 3 months,P >0.05,the difference was not statistically significant.The control group was followed up for 3 months after treatment and after treatment,P <0.01,the difference was statistically significant.2.Compared to that before treatment,the Kofoed scores of the two groups of P >0.05,the difference between the two groups was not statistically significant;March after treatment and after treatment two groups Kofoed pain score increased,both of the two group before and after treatment Kofoed score,P <0.01,there were statistically significant differences between the two groups,and compared to P <0.01,the difference between the two groups was statistically significant;comparison for the treatment group in March after treatment and follow-up after treatment of P >0.05,the difference between the two groups was not statistically significant,but compared to the control group in March after treatment and follow-up after treatment of P <0.01,the difference between the two groups was statistically significant.3.Comparing the before and after treatments,the total effective rate of the control group was 84.21%;the total effective rate of the treatment group was 94.74%,and the total effective rate of the two groups was P <0.05,and the difference between the two groups was statistically significant.Conclusion The results of the study indicate that sodium hyaluronate and triamcinolone acetonide acetate used together with Traditional Chinese Medicine herbal fumigation therapy for the treatment of AOA(treatment group)was superior to using sodium hyaluronate and triamcinolone acetonide acetate for intraarticular injection of ankle joints alone(control group)to improve patients symptoms,relieve ankle pain,and improve ankle function.Furthermore,the long-term efficacy of the treatment group was superior to that of the control group,demonstrating that the intra-articular injection combined with TCM herbal fumigation therapy was effective in the treatment of AOA. |
