| Objective:The purpose of this study is to objectively evaluate the effect of traditional Chinese medicine "Tong Yang Huo Xue Li Shui" on the treatment of acute heart failure by clinical efficacy.Methods:Randomized grouping method is adopted.All the patients in the paper were sourced from the Affiliated Hospital of Nanjing University of Chinese Medicine from May 2017 to March 2018.60 patients who met the diagnostic criteria of acute heart failure(including patients with acute decompensation of chronic heart failure,including BNP?100ng/L)and of Traditional Chinese Medicine for heart failure were divided into treatment group and control group.According to the 2014 edition of The Guidelines For Diagnosis And Treatment Of Chinese Heart Failure,both groups were treated with diuretics,vasodilatoras,positive inotropic drugs,vasoconstricting drugs as active basic treatment,and ?-blockers,aldosterone antagonists,ACEI or ARB were given according to their condition.At the same time,medicines were actively adopted to improve prognosis,and the treatment of primary diseases and related complications were made,such as lipid regulation and stable plaque in patients with angina pectoris,infection control in infected patients,anticoagulant in patients with atrial fibrillation,so as to reduce or eliminate a variety of incentives.The treatment group was treated with "Tong Yang Huo Xue Li Shui" prescription,one dose a day,200ml,orally in the morning and evening,the course of treatment was 7 days.By observing and recording the scores,the grade of severity,the grading and quantifying scores on the symptoms of selected patients before and after treatment,the effect of TCM syndrome,BNP levels,the changes of cTNI,24h urine volume and heart function,the numbers of patients with recurrent heart failure and readmission within 30 days were used as the estimation and evaluation indicators.Results:1.There was no statistical difference in acute heart failure scores between the two groups before treatment(P>0.05).The acute heart failure scores of the same group decreased significantly after treatment(P<0.01).But the comparison between the two groups on the 7th day showed no statistical difference(P>0.05).2.There was no statistical difference in the grading and quantifying scores on the symptoms of Traditional Chinese Medicine between the two groups before treatment(P>0.05)· After treatment,the scores in the two groups decreased(P<0.01).The improvement of the scores in the treatment group was better than that in the control group(P<0.01).On the curative effect of Traditional Chinese Medicine syndrome,the total effective rates of the treatment group and the control group were 93.3%and 66.7 respectively(P<0.05).3.There was no statistical difference in BNP level between the two groups before treatment(P>0.05).The BNP values of the same group decreased significantly after treatment(P<0.05).And the comparison between the two groups on the 7th day showed statistical difference(P<0.05).4.There was no statistical difiference in serum troponin I values between the two groups before treatment(P>0.05).After treatment there was statistical difference between the treatment group and the control group(P<0.05).5.There was no statistical difference in 24h urine volume between the two groups before treatment(P>0.05).The 24h urine volume values of the same group decreased after treatment(P<0.05),and there was statistical difference between the treatment group and the control group(P<0.05).6.There was no statistical difference in comparative LVEF and LVIDd between the two groups at before and after treatment(P>0.05).7.There was statistical difference in the numbers of patients with recurrent heart failure and readmission between the two groups within 30 days(P<0.05).Conclusion:Both the control group and the treatment group could alleviate the scores and the clinical symptoms of acute heart failure,improve the effect of TCM Syndrome,reduce the value of BNP and cTNI and increase the amount of 24h urine,the improvement of 24h urine volume and reduce the value of BNP and cTNI in the treatment group was better than that in the control group,At the same time,the treatment group was superior to the control group in the numbers of patients with recurrent heart failure and readmission within 30 days. |
