Background and ObjectiveGlossopharyngeal neuralgia(GPN)is a rare clinical facial pain syndrome characterized by paroxysms of torturous pain in the sensory distribution area of the auricular and pharyngeal branches of glossopharyngeal nerves.Usually,GPN manifests with a deep stabbing pain or shock-like pain on one side of the throat,root of tongue,mandibular area and tonsillar area,sometimes radiating into the ear.The patient often causes symptoms by swallowing,chewing,coughing,speaking,opening mouth,etc.The aim of this study was to investigate the efficacy of laterocervical approach for glossopharyngeal neurotomy to treat idiopathic glossopharyngeal neuralgia,and its advantages and disadvantages.MethodsA retrospective analysis was undertaken for 76 patients with GPN form July 2006 to July 2017 at the First Affiliated of Zhengzhou University.Follow-up data was collected through interview.Results71 patients of the cases were followed-up for 6 months to 138 months with an median of 52 months.Among the 71 patients suffered from glossopharyngeal neuralgia,66 patients were satisfied with the outcomes and did not recur until the end of follow-up(cure rate 92.96%).2 of 71 patients were relapsed in our study,and the relapse rate is 2.82%.The immediate remission rate of pain Was 71.83%,and the complete remission rate of pain Was 95.77%.Intraoperative leakage of cerebrospinal fluid in 3 cases and postoperative cerebrospinal leak occurred in 16 cases.Postoperatively,foreign body sensation of pharyngeal area in 6 cases,slight dysgeusia in 2cases,choking in 2 cases,dysphagia in 2 cases,hoarseness in 2 cases,facial palsy was found in 3 cases,food backstreaming into the nasal cavity in 1 case,no death.conclusionLaterocervical approach for glossopharyngeal neurotomy is a safe,available method which with high cure rate and slight complication to treat idiopathic glossopharyngeal neuralgia.
