Objective:Abnormal uterine bleeding is a common disease in gynecological patients.The causes are complex,and there are different reasons for the different age groups.Vaginal ultrasonography and diagnostic curettage are common methods for diagnosis and treatment of abnormal uterine bleeding.But the rate of missed diagnosis is high,and the cause can not be identified in a timely manner,which can delay the disease and increase the psychological and economic burden of the patients.Hysteroscopy is a kind of advanced gynecological minimally invasive diagnosis and treatment technique developed in recent years,and has been applied in diagnosis and treatment of uterine cavity lesions.This research through the retrospective analysis the clinical features of patients with abnormal uterine bleeding and make a diagnosis and give treatment methods,to explore the value of hysteroscopy in diagnosis and treatment of abnormal uterine bleeding,to the early diagnosis and treatment of abnormal uterine bleeding provide certain scientific basis.Methods:To collect clinical data of 332 patients treated for uterine bleeding,According to the pathological results,264 cases of abnormal uterine bleeding were included in the study.By comparing vaginal ultrasonography and hysteroscopy in intrauterine lesion detection rate and the cause of abnormal uterine bleeding detection rate,the sensitivity,specificity,and negative predictive value and positive predictive value of contrast,analysis of the differences.Results:1.The age distribution characteristics of patients with abnormal uterine bleeding: the results showed that the majority of patients with abnormal uterine bleeding were 36 years old to perimenopausal women.2.Hysteroscopy examination was compared with the vaginal ultrasound in the diagnosis of intrauterine lesions: hysteroscopy examination for histopathological diagnosis criteria,specificity,and negative predictive value in terms of intrauterine lesions were significantly different(p < 0.05),hysteroscopy examination and vaginal ultrasound sensitivity and positive predictive value is higher,there was no statistically significant difference(p > 0.05).3.Hysteroscopy and vaginal ultrasound examination of endometrial polyps(diagnostic criteria for histopathological findings).The sensitivity of hysteroscopy to endometrial polyps(95.1%)and negative predictive value(91.4%),the sensitivity of vaginal ultrasound in the detection of endometrial polyps was 81.5%,and the negative predictive value was 67.2%.The difference between the sensitivity and negative predictive value of endometrial polyps was statistically significant(p < 0.05).Hysteroscopy in the detection of endometrial polyps(92.1%),specificity,positive predictive value(91.4%),vaginal ultrasound in the detection of endometrial polyps of the specificity of 84.4%,positive predictive value of 79.7%,both in the detection of endometrial polyps of specificity and positive predictive value are high,there was no statistically significant difference(p > 0.05).4.In the case of pathological findings,264 cases were diagnosed with abnormal uterine bleeding,five of which were malignant lesions,and malignant lesions accounted for 1.89%,mainly benign lesions.Conclusions:1.Hysteroscopy compared with vaginal ultrasound,both in the diagnosis of intrauterine lesions has higher sensitivity and positive predictive value,vaginal ultrasound has no traumatic characteristics can be used as routine screening inspection methods of intrauterine lesions.2.Hysteroscopy compared with vaginal ultrasonography and hysteroscopy in the diagnosis of abnormal uterine bleeding compared with transvaginal ultrasound,the sensitivity,specificity,and negative predictive value are relatively high,The suspicious cases,hysteroscopy examination is very necessary.
