Objective: 1.To observe the clinical characteristics of amnestic mild cognitive impairment in patients with type 2 diabetes mellitus(T2DM-aMCI)and to analyse the related risk factors of T2DM-aMCI.2.To explore the diffenerce of cognitive impairment between T2DM-aMCI and nonT2DM-aMCI.Methods:1.This is a cross-sectional study.A total of 112 patients with type 2 diabetes mellitus were collected by diagnosis and classification criteria of diabetes mellitus in WHO1999 and inclusion criteria.They came from the department of neurology and endocrine in the Hospital of Southwest Medical University from 2016.1 to 2017.12.They were divided into two groups:T2DM-aMCI group(n=57)and T2DM-NMCI group(n=55)by Petersen clinical diagnostic criteria in 1999.Then analysing the clinical characteristics of T2DM-aMCI.Risk factors which may associate with T2DM-aMCI were analyzed,including basic data,vascular risk factors and diabetes related indexes.Multivariate analysis was carried out on the risk factors which existed difference between two groups.Then finding the independent risk factors for T2DM-aMCI after correcting confounding factors.2.A case-control study was applied to explore the difference of cognitive deficts between T2DM-aMCI group and nonT2DM-aMCI group.Compared case group(57 cases of T2DM-aMCI)with control group(55cases of nonT2DM-aMCI),age,gender and level of education were matched.Results:1.The proportion of T2DM-a MCI were impaired in visual-spatial /executive function,attention,language,abstraction,delayed memory and calculation(98.2%,64.9%,24.6%,89.5%,100%,49.1%)were respectively higher than those of T2DM-NMCI group(51.1%,14.5%,5.5%,20.0%,18.2%,18.2%)and with statistic significance(P<0.05).The scores of visual-spatial/executive function,attention,language,abstraction,delayed memory and calculation in T2DM-aMCI group were 2.84±0.77,2.35±0.48,2.75±0.43,0.57±0.58,3.15±0.52,2.36±0.72,which were respectively lower than those of T2DM-NMCI group(4.41±0.76,2.83±0.42,2.94±0.22,1.78±0.45,4.81±0.38,2.78±0.49),there was significant difference in two groups(P<0.05).T2DM-aMCI group compared with T2DM-NMCI group,the risk factor showed age,hyperlipidemia,course of diabetes,blood glucose control level,diabeticmicrovascular complications(diabetic retinopathy)and hypoglycemia were significantly different(P<0.05).To adjust confounding factors by the logistic regression,the result revealed age was independent risk fator in T2DM-aMCI,the OR(95.0%CI)were1.159(1.051-1.278).2.The proportion of T2DM-aMCI were impaired in attention(64.9%),abstraction(89.5%)and calculation(49.1%)were respectively higher than those in nonT2DM-aMCI group(25.5%,54.5%,23.6%),the difference was conspicuously significant(P<0.05),and theproportion of delayed memory impairment in two groups were 100%.The scores of attention,abstraction and calculation(2.35 ±0.48,0.57±0.58,2.36 ± 0.72)in T2DM-aMCI group were respectively lower than nonT2DM-aMCI group(2.72±0.48,1.23±0.79,2.70±0.56).Attention,abstraction and calculation were impaired more severely in T2DM-aMCI(P<0.05).Conclusions:1.The visual-spatial/executive function,attention,language,abstraction,delayed memory and calculation were impaired in T2DM-aMCI.2.Age,hyperlipidemia,course of diabetes,blood glucose control level,diabetic microvascular complications(diabetic retinopathy)and hypoglycemia were associated with T2DM-aMCI.Age was independent risk factor of T2DM-aMCI.3.Except for delayed memory,the difference of cognitive deficts between T2DM-aMCI and nonT2DM-aMCI was in abstraction,attention and calculation.Attention,abstraction and calculation were impaired more severely in T2DM-aMCI.
