Objective To analyze the value of Alzheimer-associated neuronal thread protein?AD7c-NTP?levels by measuring them in patients with amnestic mild cognitive impairment?aMCI?and vascular mild cognitive?VaMCI?.Methods Seventy patients with MCI were selected from April 2017 to October 2017in Anhui Provincial Hospital of Neurology aged 50 to 85 years old.They were divided into two groups:aMCI group?n=40?and Va MCI group?n=30?.In the same period,we selected 27 normal subjects who had normal cognitive function tests at our physical examination center,aged 50 to 84 years old.All subjects were collected for general clinical data.Then they were systematically tested in laboratory and imaging.All subjects were enrolled in the early urination of the first urine.The qualified specimens were tested for the level of AD7c-NTP by enzyme-linked immunosorbent assay?ELISA?,and the differences of urinary AD7c-NTP levels in each group were compared.All subjects were tested by neuropsychological scale.The relationship between MMSE and Mo CA scores and urinary AD7c-NTP levels were statistically analyzed.And the diagnostic sensitivity and specificity of aMCI were analyzed by ROC curve analysis of urine AD7c-NTP level.Results 1.The levels of urinary AD7c-NTP in aMCI group,VaMCI group and control group were?3 427.63±1 126.25?ng/L,?2 789.76±974.04?ng/L,?2 289.95±656.98?ng/L.The urinary AD7c-NTP level in urine was significantly higher than that in VaMCI group and control group?P<0.05?,while the urinary AD7c-NTP level in VaMCI group was higher than that of control group?P>0.05?.2.The scores of the aMCI group and the control group in each area of the MMSE scale were compared,and the scores of the aMCI group were lower in terms of delayed recall and language?P<0.05?.While compared with the control group,the VaMCI group had a decrease in delayed recall,language,and visual perception?P<0.05?.The scores in the aMCI group compared with the control group in the MoCA scale field were significantly lower in the aMCI group in terms of visuospatial/executive,attention,language,and delayed recall?P<0.05?.However,compared with the control group,the VaMCI group showed a decrease in visual space and performance ability,language,delayed recall and orientation?P<0.05?.3.The level of urinary AD7c-NTP was negatively correlated with MMSE score and MoCA score?r1=-0.278,P1=0.006,r2=-0.362,P2<0.01?.After correcting the impact of education years,urinary AD7c-NTP levels are still related to MMSE score and MoCA score?r1=-0.283,P1=0.005,r2=-0.366,P2<0.01?.4.Using receiver operating characteristic?ROC?curve,the optimal cutoff point of AD7c-NTP in urine for diagnosis of aMCI was 2 507.10 ng/L,with 80%sensitivity and 77.8%specificity,the area under the curve was 0.828?95%CI:0.7280.928?.In both urine AD7c-NTP levels of 2 507.10 ng/L,the diagnosis of aMCI sensitivity and specificity of the highest.Conclusion AD7c-NTP levels in the urine for the diagnosis of MCI,especially aMCI have some clinical value,and there may be some correlation between the severity of aMCI.
