Objective To compare the efficacy,safety and cost-effectiveness between endoscopic resection(ER)and traditional surgery for the treatment of early gastric cancer and precancerous lesions.Methods A total of 141 early gastric cancer or precancerous patients who had been admitted to the Second Affiliated Hospital of Nanjing Medical University during January 2014 to December 2016 were included in this study.Parameters of each patient,such as age,sex,symptoms,time of hospitalization,total treatment cost and incidence of complications,were collected and compared in detail.Results A total of 106 patients were included in the ER group and 35 in the traditional surgery group.The mean diameter of lesions from two groups were 1.8±1.0cm,2.2±0.9cm.The complete resection rate in the traditional surgery group was slightly higher than that of the ER group(97.1% vs 95.3%,P= 1.000),however,the incidence of complication was significantly higher in the traditional surgery group(20% vs 4.7%,P = 0.014).The ER group showed a shorter mean hospitalization time(d)(9.6 vs 18.4,P<0.001)and lower mean treatment cost(RMB)(22854.7 vs66905.6,P<0.001)than the traditional surgery group.Conclusion ER and surgery have equivalent efficacy in treating early gastric cancer and precancerous lesions,but ER should be the first choice because of its shorter hospital stays,lower costs,ewer complications.
