The Value Of Combining NLR And CA-125 In The Detection Of Endometriosis

Object:To study the value of combining NLR and CA-125 in the detection of endometriosis.Methods:Select 92 patients who accepted laparoscopy or laparotomy operation in the Affiliated Suzhou Hospital of Nanjing Medical University from Janurary2016 to May 2017.43 of them are pathologically confirmed EMT,these cases are seted as a study group.49 of them are pathologically confirmed benign ovarian tumors,these cases are seted as a control group.According to the American Fertility Society classification criteria,24 of all EMT cases are classified in mild stage,19 cases in severe stage.Record all patients' data,including age,blood count and CA-125.Calculate the ratio of neutrophils to lymphocytes.Use SPSS 22.0 software to analyze the related data.Sensitivity and specificity of NLR,CA-125 and their combined marker(NLR*CA-125)are measured by receiver operating characteristic(ROC)curve.So that,we can investigate the the value of these markers in the detection of endometriosis.Result:(1)Patients' age,leucocyte count,neutrophilic granulocyte count and lymphocyte count have no statistical difference between the study and control group(P>0.05).(2)The level of NLR in the study group is higher than the control group(P<0.05);The level of CA-125 and combined marker in the study group is higher than the control group and the difference is obviously statistically significant(P<0.001).(3)In different stages of EMT,there is no statistical difference between patients' leucocyte count,neutrophilic granulocyte count and lymphocyte count(P>0.05).(4)In different stages of EMT,the level of NLR has no statistical difference(P>0.05).The level of CA-125 and combined marker is higher in severe stage and the difference is obviously statistically significant(P<0.001).(5)By using the ROC curve,the sensitivity of NLR?CA-125 and combined markers is(88.4%?65.1%?72.1%),the specificity(34.7%?93.9%?89.8%).NLR is higher than CA-125,combined markers in sensitivity while CA-125 and combined marker is higher in specificity.(6)The positive rate of NLR?CA-125 and combined marker is(87.5%?45.8%?58.3%)in mild stage EMT.The positive rate of NLR?CA-125 and combined markers is(94.7%?89.5%?89.5%)in severe stage EMT.In mild stage,the positive rate of NLR is higher than CA-125 and combined marker.In severe stage,the positive rates are similar.(7)In different stages of EMT,the positive rate of NLR has no significant difference(P>0.05).The positive rate of CA-125 and combined marker both are higher in severe stage(P<0.05).Conclusion:(1)Compared to CA-125,NLR has higher sensitivity,especially in detecting mild stage EMT.(2)The combined marker can help improve the accuracy in the detection of edometriosis and it has high specificity.
