Objective.In gynecologic tumors in our country, ovarian cancer is one of the highest rates of malignant tumors, due to its early symptoms were not easily found, so when confirmed, the majority of patients with lymph node involvement or distant metastasis, the 5-year survival rate is only about 30% or less (changes limited to the ovaries of the female 5-year survival rate is 80%-90%[1]). Usually appear the symptoms such as abdominal pain, abdominal discomfort, its performance is nonspecific symptoms, so can’t accordingly in early diagnosis of disease, diagnosis usually has to late so the prognosis is poor. In addition, the diagnosis of ovarian cancer need surgery; However, many of the pelvic mass surgery results confirmed benign accessory bag piece. If you can early diagnosis and timely treatment of ovarian cancer, to improve the prognosis of patients with ovarian cancer has very important clinical significance. At present the application of serum biomarkers to diagnose ovarian cancer is a hot area of research. A lot of ovarian cancer biomarkers are still in research stage. Reported from the serum markers may be useful are as follows:carbohydrate antigen 125, human epididymis protein (human epididymis protein 4, HE4), carcinoembryonic antigen (CEA), a tire protein (AFP), osteopontin, mesothelium element, lysophosphatide acid (lysophosphatidic acid, LPA), haptoglobin, thyroxine transporters, apolipoprotein Al, serum c-reactive protein, etc. Biomarkers or biomarker combination is often used in combination, or with other methods, such as ultrasonic) share. Carbohydrate antigen 125 is the focus of most of the research, also is the most widely used. Currently only with a single CA125 in the diagnosis of early ovarian cancer diagnosis rate is lower, because of CA125 in premenopausal women benign condition (such as menstruation, uterine fibroids or ovulation) serum biomarkers has increased. Therefore, a variety of combination joint application of serum biomarkers, in order to improve the early diagnosis of ovarian cancer specificity, sensitivity, is a hotspot of current research. This article preliminarily HE4 and CA125, CEA and AFP four kinds of serum tumor markers combined detection of applied value in the diagnosis of ovarian tumors.Methods:selected 80 patients with benign ovarian tumors dong’e hospital,40 patients with ovarian cancer and healthy control group 80 cases all were CA125, HE4, four kinds of serum tumor markers CEA and AFP for joint detection, finally data using SPSS 17.0 software to carry on the t test, analysis of various index statistical differences between groups. P< 0.05 statistically significant.Results:the ovarian cancer groups (single diagnosis positive rate from high to low of HE4 respectively 80%(32/40),80.00%(32/40), CA125 (9/40), AFP15 CEA22.50%%(6/40). Ovarian cancer group and the control group with significant difference. Compared with the control group, HE4 and CA125 Kappa index were 0.7,0.842, while CEA and AFP two index Kappa index was 0.279 and 0.279, respectively, the sensitivity is low, and the missed diagnosis is high, the individual test for clinical application of malignant tumors can cause adverse consequences. Compared with benign ovarian tumor group and the control group, except the HE4 has no obvious difference. Ovarian cancer and benign tumor group comparison of HE4 and CA125 Kappa index were 0.602,0.683, (P=0.001), two methods of diagnosis for moderate consistency, satisfied. Individual serum tumor markers in the diagnosis of ovarian cancer, the sensitivity of HE4 up to 87.5.00%, followed by CA125 (85%). Using the four indexes combined analysis, diagnosis efficiency to 92.5%, the sensitivity and specificity were 92.50% and 76.67% respectively. Joint detection compared with the single index detection, sensitivity and efficiency of diagnosis are improved significantly.Conclusion:the combined detection of serum CA125, HE4, CEA and AFP can improve the positive rate of ovarian cancer detection, and improve the efficiency of its early diagnosis. Four indicators help to reduce the misdiagnosis and false negative and joint detection, improve the accuracy of the ovarian malignant tumor diagnosis, early diagnosis of ovarian malignant tumors have certain value.
