Clinical Experience Of Professor Li Jiakang In The Treatment Of Lumbar Intervertebral Disc Herniation Based On Data Mining

Objective: To investigate the key elements of clinical cases through data processing,and to find out the dialectical thinking and diagnosis and treatment rules of professor Li jiakang's treatment of lumbar intervertebral disc herniation(LDH),and to summarize and explore the analysis.Research methods: Research materials were selected from September 2015 to February 2018,including outpatient records and past ward records of Professor Li Jiakang's rounds,standardize the terms of traditional Chinese medicine and make an electronic medical record template,entry of the medical records into the integrated system of clinical scientific research and the data is extracted from the system,after preliminary data screening,the LDH database was generated according to the patient's name,sex,date of birth,tongue,tongue coating,pulse image,disease name,TCM syndrome,acupoint selection,drug composition and unit drug dosage.Using SPSS24.0software and the data mining analysis function in the V1.3.7 software of ancient and modern medical case cloud platform,the frequency analysis of the patient's name,sex,symptom,sign,meridian,acupoint and drug use is carried out to find out the basic clinical features of the LDH patients and the points and prescriptions commonly used by Professor Li Jiakang to treat the disease,then we use data mining and analysis function to do association rule analysis and mutual informationanalysis,and get the combination rule of Li Jia Kang's combination of acupoint and medication,then,a cluster analysis and complex network analysis of acupoints and traditional Chinese medicine in the database were obtained,and the core acupuncture and TCM prescriptions of professor li jiakang in the treatment of LDH were obtained.Finally,we summed up the characteristics of Professor Li Jiakang's selection of acupoints and the use of traditional Chinese medicine in the treatment of LDH.Results:1.General situation: The study included 854 cases.There were277 males and 577 females.Age are mainly distributed in 41-80 years old.2.The results showed that the most clinically significant syndromes were liver and kidney deficiency type,the frequency of occurrence is622,accounting for 54.28%,Secondly,qi stagnation blood stasis type,cold-damp coagulation type;The disease is most commonly seen in the bladder,the frequency of occurrence was 576 times,accounting for68.33%;The first 10 points in clinical use are Shenshu,Dachangshu,Qihaishu,Huantiao,Weizhong,Yaoyangkuan,Guanyuan,Jiaji point,Pishu,Tsusanli;The acupuncture points were analyzed,and the number of acupuncture points on the bladder was found to be the most,with 6178 frequent occurrence frequency,23 acupuncture points were selected,the second is gallbladder meridian and spleen meridian;The position of acupuncture is mainly on the back of the waist;The highest frequency of Chinese angelica was 236 times,the average dose was10.62 grams,the minimum dose was 6 grams,the maximum dose was50 grams,and the second was the root of fangfeng and the fry radix paeoniae alba;According to the statistics of the properties of medicinalmaterials,it was found that the most commonly used drugs were warm,flat,sweet and spicy drugs.3.Effect analysis: The effect of outpatient treatment for more than two courses was evaluated,the VAS scale and the JOA score table were used in the evaluation,the total of 103 patients were evaluated,according to the grading curative effect evaluation,after 2 courses of treatment,43 cases were excellent,52 good,8 fair,and 0 poor,the total effective rate was 100%,and the markedly effective rate was 93%.4.Correlation analysis: The following are the high frequency of acupoint compatibility: Shenshu-Dachangshu,Shenshu-Qihaishu,Shenshu-Yaoyangguan,Shenshu-Jiaji,Shenshu-weizhong;The following is the higher frequency of drug compatibility: Radix angelicae tuhuo-The root of fangfeng,Ligusticum wallichii-Chinese angelica,Parasitic loranthus-Radix angelicae tuhuo;The relationship between the following syndromes and the acupoint relationship is strong: The correlation between Shenshu,Dachangshu,Guan yuan,Jiaji point and Juegu is strongly correlated with Liver-Kidney Deficiency Syndrome,the correlation between Qihai,Weizhong,Yaoyangguan,Ciliao is strongly correlated with Qi stagnation and blood stasis type.5.Cluster analysis: Acupoints were clustered into 5 categories:C1: Shenshu,Dachangshu,Qihai ShuC2: Yao JiajiC3: Yaoyangguan and ZusanliC4: Ganshu,Pishu,Yinlingquan,Sanyinjiao,Ciliao,Guanyuan Shu,Yaoyi,Huangmen,Sanjiao Shu,ZhishiC5: Weizhong,Heyang,Huantiao,ZhibianThe drugs were clustered into 6 categories:C1: Radix rehmanniae,Achyranthes bidentata,kachyranthesbidentata and fry HawthornC2: Salvia miltiorrhiza,Yan HusuoC3: Codonopsis pilosula,Atractylodes macrocephala and AstragalusC4: Cinnamon branchC5: LicoriceC6: Fangfeng,angelica,angelica,chuanxiong,Radix Paeoniae Alba,Herba taxilli,eucommia bark,tangerine peel,Poria cocos6.Complex network analysis: Taking the acupoints as nodes,the number of two acupoints appearing in the same prescription is taken as the weight of the edge,by setting the minimum weight value,we filtrate the low frequency acupoints,draw the complex network diagram of high frequency points,and get the core acupuncture prescription:Dachangshu,Shenshu,Qihaishu,Yaoyangguan,Weizhong;Similarly we can get the core of prescription drugs: Angelica,angelica,Radix Saposhnikoviae,Rhizoma Chuanxiong,Radix Paeoniae alba.Conclusions:1.Professor Li jiakang's treatment of lumbar intervertebral disc herniation is rapid and effective.2.Professor Li Jiakang's understanding of etiology and pathogenesis of LDH is that it is a disease caused by asthenia in origin and asthenia in superficiality,mainly due to deficiency of liver and kidney,qi-stagnancy and blood stasis.3.Professor Li jiakang's basic treatment of lumbar intervertebral disc herniation is "Invigorating kidney and Removing blood stasis".4.Professor Li Jiakang's choice of acupoints is based on the Discrimination of meridian and collaterals,the combination of the distant-local point," Warming Yang and dispelling cold,Stimulate thecirculation of the blood and cause the muscles and joints to relax,Removing stasis and relieving pain " as the basic method.5.Professor Li Jiakang clinically uses drugs to treat both the symptoms and root cause,tonification and purgation in combination,and balance between Qi and deficiency.
