The Study Of The Construction Of Management Mode Of Chronic Disease In Community Pharmacy

Objective:This article analyzes the current situation and difficulties in the management of chronic diseases in Chinese community pharmacies through an empirical research,and studies the theoretical research foundation and successful practice experience of the chronic disease management mode in foreign community pharmacies.Put forward the countermeasures and suggestions of the construction of the management mode of chronic disease in Chinese community pharmacies.And explore the management mode of chronic disease which is suitable for community pharmacies in our country.To resolve the major public health problems in China,and promote the social pharmacy service level,and safeguard people's health.Methods:On the basis of literature analysis,this paper systematically cards the research achievements on the management of chronic diseases in community pharmacies and the management of chronic diseases in China and abroad.We choose 20 pharmacists who are responsible for the management of diabetes to ask some questions around the pharmacists service project implementation and service implementation effect,the management of chronic diseases in community pharmacies mainly faces obstacles,chronic disease service training and services to enhance the ability to carry out four aspects of insider interviews and randomly selected 50 cases of patients in community pharmacies with diabetes health records,construct statistical research descriptive.Combined with the classic cases of chronic disease management in community pharmacy at home and abroad,this paper makes a comparison and analysis,and summarizes the experience for reference in China.Result:At present,the community pharmacy in our country mainly graft the specialized diabetes pharmacies into the traditional community pharmacy in the form of establishing the chronic disease management project with the third party.However,the scale of the community pharmacy has begun to take shape,but there is still poor compliance of patients with drugs.Pharmacists lack of professional knowledge,irrational use of drugs,slow disease service input costs,short-term benefits difficult and so on.In the course of constructing the management mode of chronic disease in community pharmacy of our country,we can draw lessons from the valuable experience of the management of chronic disease abroad,and establish and perfect the management policy of chronic disease in social pharmacy of our country on the basis of introducing and perfecting the management policy of chronic disease in social pharmacy of our country.Actively promote the development of "Internet" chronic disease management in China,with professional medical and health care institutions,large number According to the cooperation of enterprises,further improve the quality of community pharmacy chronic disease management services.Conclusion:The construction of chronic disease management mode in Chinese community pharmacy should be carried out from two perspectives of macro and micro.Macro policy suggestions are:to establish the value concept of chronic disease management;carry out the classification of the service strategy of pre intervention,prevention of chronic disease;strengthen the drug monitoring system construction,improve the service level of chronic disease;chronic disease management to promote health propaganda,enhance the professional image of community pharmacy.The microscopic perspective mainly provides effective policy recommendations for the development of chronic disease management in our social pharmacy from six aspects:the value concept of chronic disease management,software configuration,hardware configuration,service standard,supervision and management,and the promotion of chronic disease management of Chinese community pharmacy.
