Research Thinking Of The Management Of Chronic Kidney Disease With Tongfu Xiezhuo Therapy Based On The Theory Of Gut-kidney Axis

Objective:Based on the model of gut-kidney axis theory,this study explored the effect of TongFu XieZhuo Therapy on renal failure rats with intestinal bacteria and the effect of down-regulating metabolic toxin levels,to evaluate the animals' therapeutic effects on renal failure rats,and to further explore the related mechanism for delaying the progression of chronic renal failure through the method of TongFu XieZhuo Therapy..Methods:This study was divided into three parts:1)Effect of TongFu XieZhuo Therapy on uremia toxin in chronic kidney disease rats;2)Effect of TongFu XieZhuo Therapy improves renal fibrosis in chronic kidney disease rats.3)Effect of TongFu XieZhuo Therapy improves intestinal flora imbalance in chronic kidney disease rats.Part 1:24 experimental rats were divided in to blank group,TongFu Xiezhuo decoction group,model group and Peritoneal dialysis group.All rats model with chronic kidney disease by adenine(200mg·kg-1·d-1)49 days,and then intragastrically administrated once a day except the blank group.After 56 days' treatment,the urea nitrogen,creatinine,uric acid and cystatin in serum were measured,and the content of indoxyl sulfate in serum was determined by HPLC-MS.Part 2:24 experimental rats were divided in to blank group,TongFu Xiezhuo decoction group,model group and Peritoneal dialysis group.All rats model with chronic kidney disease by adenine(200mg·kg-1·d-1)49 days,and then intragastrically administrated once a day except the blank group.After 56 days' treatment,the TIFI and TGF-?1 of renal were measured.Part 3:30 experimental rats were divided in to blank group,Xiezhuo decoction group,model group and rhubard group.All rats model with chronic kidney disease by adenine(200mg·kg-1·d-1)49 days,and then intragastrically administrated once a day except the blank group.After 56 days'treatment,the urea nitrogen,creatinine,uric acid,IL-6 and cystatin in serum were measured,and the content of indoxyl sulfate in serum was determined by HPLC-MS.All rats measured the content of bifidobacterium and colibacillus by 16srDNA-based fluorescent real-time PCR.The expression of TGF-?1 protein in renal tissues were detected by immunohistochemistry.Results:Part 1:The content of urea nitrogen,creatinine,uric acid,cystatin and indoxyl sulfate in serum were decreased in the Xiezhuo decoction group(P<0.05)Moreover,the Xiezhuo decoction group were better than Peritoneal dialysis group(P<0.05).Part 2:The content of TIFI and TGF-?1 were decreased in the Xiezhuo decoction group(P<0.05)Moreover,the Xiezhuo decoction group were better than Peritoneal dialysis group(P<0.05).Part 3:The content of urea nitrogen,creatinine,uric acid,cystatin,IL-6,TGF-?1,and indoxyl sulfate were decreased in the Xiezhuo decoction group(P<0.05).The content of bifidobacterium was rised,and colibacillus was decreased in the Xiezhuo decoction group(P<0.05).Conclusions:1."Retention of Turbidity Toxin in the Interior" is the basic pathogenesis of Traditional Chinese Medicine in chronic renal failure.2.Application of TongFu Xiezhuo Therapy had experimental efficacy,that can improve the clinical symptoms,down regulate metabolic toxin levels especially enterogenous toxin IS levels,improve renal function,thus delay the progress of renal failure.3.The therapeutic mechanism of the TongFu Xiezhuo Therapy is associated with its roles of increasing intestinal metabolic wastes and toxin excretion,regulating intestinal flora and Improving intestinal barrier function,reducing the absorption of endotoxin,or increasing the intestinal probiotics that decompositing andusing more nitrogen waste.
