Clinical Observation On Treatment Of Chronic Pulmonary Heart Disease With Heart Failure (Syndrome Due To Yang Deficiency) Based On "Lung-regulating Water Passage"

Objective:this study is based on "lung-regulating water passage" the guidance of theories,using MahuangFuziXixin(MFX)decoction having the effect of warming Yang to promote diuresis?freeing lung and relieving asthma on patients with cor pulmonale heart failure,as a clinical observation,through the NYHA heart function classification,six-minute walking test,LEE's heart failure integration,TCM syndrome integration,and the NT-proBNP index observation,validation application warming Yang and Freeing Lung,water treatment effectiveness of cor pulmonale heart failure(syndrome due to Yang deficiency),confirm the efficacy and safety of dynasties MFX decoction provide a evidence-based medicine for syndrome in traditional Chinese medicine treatment of cor pulmonale heart failure.Methods: In this study,62 patients who met the observation requirements were randomly divided into treatment group and control group.The proportion of the two groups was 1:1,with 31 cases in the treatment group and 31 in the control group.The control group was treated with standard western medicine(Aldosterone antagonists:Spironolactone tablets,Angiotensin converting enzyme inhibitors: Perindopril tablets,rational choice of antibiotics according to the drug sensitivity test).The treatment group was treated with ephedrine plus decoction,and two weeks was a course of treatment,and this subject was studied for one year.The curative effect of the experimental group and control group was evaluated by the changes of NYHA cardiac function classification,six-minute walking test,LEE's heart failure integration,TCM syndrome integration and NT-ProBNP.Treatment group and control group patients before treatment in the general information,there were no obvious difference on vital signs,safety index,NYHA heart function classification,six-minutes walking test,LEE's heart failure integral,TCM syndrome integration and NT-ProBNP,examined the statistical P values > 0.05 no statistical significance,the follow-up treatment comparable.Results: After two weeks of clinical observation,The clinical data of the treatment group and the control group were collected and analyzed with statistical analysis.The results were as follows:1,In the aspect of improving cardiac function: the treatment group and control group both can improve cardiac function,the total effective rate were 87.09% and74.19% respectively,through the rank and inspection,P = 0.046 < 0.05,the difference was statistically significant,the curative effect of treatment group is better than that of control group in improving cardiac function aspects.2,In the aspect of exercise endurance: the treatment group and control group before and after treatment by paired sample t test,P values are less than 0.01,the difference is statistically significant,both groups can increase exercise endurance,increase six-minute walking distance,through the rank and inspection,P = 0.029 <0.05,the difference was statistically significant,the curative effect of treatment group is better than that of control group in improving cardiac function aspects.3,LEE's heart failure integration: the treatment group and control group before and after treatment by paired sample t test,P values are less than 0.01,the difference is statistically significant,the two groups were significantly lower in patients with heart failure integral,two group total effective rate were 96.77% and 90.32%respectively,through the rank and inspection,P = 0.017 < 0.05,the difference was statistically significant,indicating in reducing integral Lee's heart failure the curative effect of treatment group is better than that in control group.4,TCM syndrome integration curative effect: the treatment group and control group before and after treatment by paired sample t test,P values are less than0.01,the difference is statistically significant,TCM syndrome integral were significantly decreased in both groups,are effective in improving symptoms,two group total effective rate were 96.77% and 77.42% respectively,through the rank and inspection,P = 0.020 < 0.05,the difference was statistically significant,suggesting the curative effect of treatment group is better than that in control group in improving symptoms.5,NT-ProBNP curative effect: both groups showed a significant decrease in NT-ProBNP after treatment,indicating that both the treatment group and the control group were effective in reducing NT-ProBNP.After comparing the two groups of NT-ProBNP after treatment,the t-test was statistically matched with the sample t,and the P value was less than 0.05,and the two groups were different in reducing NT-ProBNP,and the treatment group was better than the control group.6,Safety indicators: review safety indexes of two groups of patients after treatment(ALT,AST,CR,UREA),after statistics paired sample t test,the test P values were greater than 0.05,no statistical significance,there were no significant differences before and after treatment.Conclusion:Using MFX decoction having the effect of warming Yang and freeing lung to promote diuresis ? and relieving asthma combined western medicine treatment of cor pulmonale heart failure(syndrome due to Yang deficiency)to further improve the patient's heart function,increase exercise endurance,relieve symptoms and signs,reduce the NT-proBNP,and during the period of treatment is safe and worthy of clinical use.
