ObjectivesTo make comparative analysis on the factors influencing IBS-D and investigate the distribution law of the TCM syndromes of IBS-D patients with and without anxiety and depression status through the questionnaire of basic information and TCM syndromes of IBS-D patients with and without anxiety and depression status;to analyze the IFN-r and IL-6 of IBS-D patients with anxiety and depression,normal IBS-D patients and healthy people,and to explore the relevance of the degree of anxiety and depression in IBS-D patients with IFN-r,IL-6 and TCM syndromes,so as to provide reference for the prevention and comprehensive psychosomatic treatment of IBS-D in clinical practice.MethodsThe research objects were selected among the patients and inpatients in the departments of gastroenterology and psychiatry in Henan Traditional Chinese Medicine Hospital and the Third Affiliated Hospital of Henan Traditional Chinese Medicine University as well as outpatients in the departments of gastroenterology and psychiatry in the 21 communities between May 2016 and April 2017,and the IBS-D patients and healthy physical examinees in accordance with the inclusion criteria were selected for the questionnaire,which included the filling of basic information,such as age and gender,SDS and SAS,as well as the establishment of TCM syndromes.In addition,the IFN-r and IL-6 of IBS-D patients with anxiety and depression,normal IBS-D patients and healthy people were inspected.The study is divided into two parts:Research OneFirst of all,the patients were screened in the way of questionnaire by using SDS and SAS and were divided into the research group(IBS-D with anxiety and depression status,40 cases)and the control group(IBS-D without anxiety and depression status,40 cases).The TCM syndromes of the two groups of patients were classified and statisticallyanalyzed to explore the distribution law of the TCM syndromes in IBS-D patients with and without anxiety and depression status.Research TwoFirstly,40 cases were respectively selected from the IBS-D patients with anxiety and depression status and normal IBS-D patients in research one as well as healthy physical examinees in the blank group to compare the effect of the degree of anxiety and depression on IFN-r and IL-6 and to further explore the relevance of the degree of anxiety and depression in IBS-D with IFN-r and IL-6;secondly,the IFN-r and IL-6 of IBS-D patients in the anxiety and depression group were compared to analyze the situation of serum IFN-r and IL-6 of the TCM syndromes in IBS-D patients with anxiety and depression status.The data were analyzed by SPSS19.0 statistical software.The quantitative data were expressed as mean ± standard deviation(c±s),and the counting data were expressed as N(%).The variance analysis was used to compare the data of the two groups.If there was significant difference between the two groups,LSD would be used to make afterward pairwise comparison between the data,and chi-square test would be adopted in the comparison of the categorical variable rates between the groups;the quantitative data comparison difference between the two groups was examined by the independent sample t-test or correction t-test,assuming the test level was set to0.05.Results Research One1.There were 40 IBS-D patients with anxiety and depression status,including 20 IBS-D patients with anxiety status(accounting for 50.0%),20 IBS-D patients with depression status(accounting for 32.5%)and 7 IBS-D patients with anxiety and depression status(accounting for 17.5%),and it was suggested that anxiety status was more prevalent than depression status among IBS-D patients.2.There was no significant difference in age,marital status and gender between the study group and the control group(P> 0.05),but there was significant difference in educational level and occupation(P <0.05).3.There was significant difference in the distribution of TCM syndromes in IBS-D patients with and without anxiety and depression status(P <0.01).The patients of hepatic depression had higher incidence in IBS-D with anxiety and depression status(75%),while patients of non-hepatic depression had higher incidence in normal IBS-D patients(57.5%).Research Two1.The age,gender and marital status in the observation group,control group and blank group were examined by ?2 test,with p values all greater than 0.05,and there was no significant difference in the age,gender and marital status between the three groups;the educational level and occupation in the observation group,control group and blank group were also examined by ?2 test,with p values all less than 0.05,which had significant difference and indicated that patients with educational level of junior college and above and occupation of mental work tended to be more easily to suffer anxiety and depression status.2.There was significant difference between the serum IFN-r and IL-6 of the observation group,the control group and the blank control group(P <0.05).3.The expressions of IL-6 in the peripheral blood of females in the three groups were all higher than that of males,and there was no significant difference(P <0.05);the expressions of IFN-r in the peripheral blood of females in the three groups were all higher than that of males,and there was no significant difference(P> 0.05).4.The contents of IFN-r and IL-6 with different syndrome patterns in the observation group were compared,and the contents of the hepatic depression group were significantly higher than those of the non-hepatic depression group(P <0.05).Conclusion1.Anxiety and depression status commonly existed in IBS-D patients,and it was more obvious in IBS-D patients with anxiety status.2.There was certain connection between the anxiety and depression status of IBS-D patients and educational background and occupation,patients with higher educational level and occupation of mental work tended to be more easily to suffer anxiety and depression status.3.In the TCM syndromes,IBS-D patients with anxiety and depression were mainly the type of hepatic depression,which can provide reference for its clinical diagnosis and treatment.4.The contents of serum IFN-? and IL-6 in IBS-D patients with anxiety and depression status were higher than those of normal IBS-D patients and healthy people;the serum content of IL-6 in female IBS-D patients with anxiety and depression status was higher than that of females,while the serum content of IFN-r in IBS-D patients with anxiety and depression status had limited reference for gender difference.5.From the view of the TCM syndromes,the contents of serum IFN-r and IL-6 in IBS-Dpatients with anxiety and depression status were higher than those with non-hepatic depression,indicating that there was certain relevance between cytokines and the mental state of IBS-D patients.
