Background:Vitiligo is a kind of acquired skin disease,which is more common in life.It is characterized by the loss of pigment,shown as irregular white spots on skin and mucous membrane.Vitiligo can be seen all over the world and in all ages.The prevalence rate of vitiligo in China is between 0.1% and 2.7%.It has been reported that the prevalence of vitiligo in childhood accounts for nearly 1/4 of all vitiligo patients.It can occur in various parts of the body,such as the exposed parts,which can cause damage to the image of patients,causing inferiority,shame and other bad psychology.Many scholars have studied and discussed the etiology of vitiligo,among which the genetic factors have been widely accepted.Now the hot spots focus on oxidation reaction and immune disorder.Zhejiang,Shandong,Henan,Xinjiang and other provinces have carried out related epidemiological investigation,but the relevant literature in JiangXi province is rare.Therefore,we collected the patients in our hospital and analyzed the epidemiological characteristics of vitiligo with statistical software.This research use the standards of Academy of dermatology of China Society of Integrated Traditional Chinese and Western Medicine(2014),which divide vitiligo into segmental type,non-segmental type,undifferentiated type and mixed type.This study also sets the stage and severity according to the domestic standard(2014).(see the text).Objective:To compare the pathogenesis and clinical characteristics of vitiligo between children and grown ups.The key point is to summarize the characteristics of children vitiligo in Jiangxi province.Materials and methods:All the cases were out-patients who visited from October,2016 to October,2017,and were diagnosed as vitiligo by at least one of the senior dermatologists.All patients were confirmed no blood relationship among all cases and inquired forgeneral information and epidemiology information.Finally we get 324 cases,including 102 children and 222 grown ups.The basic information and clinical features of the two groups were analyzed with SPSS22.Results:1.Among 102 children,56 were males,accounting for 54.9%,and 46 females,accounting for 45.1%.Among 222 grown ups,105 were males,accounting for47.3%,117 were females,for 52.7%.there is no statistical significance.2.The average onset age of the boys was 8.43,of girls was 6.78,of men were 33.10,of women was 36.74.Gender had no effect on the age of onset.3.The occurrence and aggravation of vitiligo are more common in spring and summer,and tend to be stable in the autumn and winter.4.For children with less than 1 year of disease,the proportion of patients with advanced stage is 77.8%;for who have been ill for more than 1 year,the ratio is 54.2%.For adults with less than 1 year of disease,the proportion of patients with advanced stage is 81.7%;for who have been ill for more than 1 year,the ratio is 69.5%.5.The incidence of anxiety and sadness was 32.4% for adults,compared with 9.8% for children.6.The number of children whose relatives got vitiligo was 13.7%;9% in adults;The difference is meaningless.Among them,most of the relatives were the I and II levels.7.There was no difference of the age between them whether with a positive or a negative of family history.8.In the initial type and current type,children are mainly segmental type,and adults are mainly non-segmental type 9.The undivided types of vitiligo could gradually changed into other ones;non-segmental and segmental ones rarely convert to each other.10 ?Children with vitiligo have the highest probability of primary skin lesions on the face,chest and lower limbs.Adults with vitiligo have the highest probability of primary skin lesions on the face,upper limbs and neck.11.More than 90% of patients' lesions did not exceed 5% of the body surface,and no more than 50% of the cases were found.12.For two groups,the probability of pruritus was similar,even with different types and stages.In adults,the area of skin lesions is larger than that of patients without itching.13.There was a 9.8% chance of developing nevus in children,which was not significantly different from that of adults.Children with nevus or not are identical in the category of vitiligo.14.The incidence of related diseases in grown-uppatients is higher than that of children,specifically chronic urticaria,hyperthyroidism or hypothyroidism,chronic hepatitis b,connective tissue disease,myasthenia gravis,et al.Conclusion:1.Children vitiligo is different from the adult vitiligo in the different genders of onset priority,the first lesion location and the clinical type.2.Children with vitiligo are less affected by mental factors than adults,and the prevalence of immune related diseases is less than that of adults.3.There was no obvious association between the halo nevus and the children vitiligo or adult vitiligo.4.Family history has no significant effect on children and adult onset,type and severity of disease.
