Objective:Acute poisoning is a disease that with acute onset and sudden progress,part of the patients can quickly develop multiple organ dysfunction syndrome(MODS)and with high mortality rate.In this study,the clinical data of patients with MODS induced by poisoning were collected to investigate the efficacy of sequential blood purification compared with single blood purification for the treatment of MODS caused by poisoning.Method:A total of 43 patients with MODS caused by poisoning admitted to Emergency Department,the first affiliated hospital of Nanchang University from Novenber 2013 to March 2018 were randomly devided into control group and treatment group.The control group was treated with single blood purification,and the treatment group was treated with sequential blood purification.Compare the following parameters between two groups: general situation,hospitalization time,hospitalization costs and mortality.The results of major biochemical tests,PSS,APACHE ? score before and after treatment of all patients were also statistical analyzed.All of the patients were followed up after discharge.Results:1.There was no statistically significant difference between the general situation betweenthe two groups(P>0.05).2.Compared with the control group,the indexes of ALT,AST,CK-MB,LDH,Cr and Bun of the treatment group were significantly reduced after 5 days of treatment,and the differences were statistically significant(P<0.05).3.Five days after treatment in treatment group,the APACHE ? score significantly lower than the control group,the difference was statistically significant(P<0.05).4.Compared with the control group,the mortality of the treatment group showed no significant decrease(P>0.05).There are two patients accompanied with chronic renal failure in the control group,no sequelae occurred in the treatment group.5.Compared with the control group,the length of hospital stay in the treatment groupwas shorter,but the difference was not statistically significant(P > 0 0.05).The hospitalization cost of treatment group was higher than the control group,and the difference was statistically significant(P<0.05).6.According to the prognosis,it's divided into two groups,the alive group and dead group,and the APACHE ? score before treatment of the death groups is higher than the alive group,the difference was statistically significant(P<0.05).The patients were also devided into PSS=2 group and PSS=3 group according to the PSS score before treatment,the mortality of the PSS=2 group was lower than the PSS =3 group,and the difference was statistically significant(P<0.05).Conclusion:1.In improving the function of organs and reducing the APACHE? score of the posioning patients with MODS,sequential blood purification treatment was superior to single blood purification treatment.2.The higher the APACHE ? and PSS scores were,the higher mortality would be,both of them supplement and confirm each other,which are helpful in assessing accurately the clinical severity and the prognosis.3.Sequential blood purification can not reduce the mortality in treating patients with MODS induced by poisoning compared with single blood purification treatment,but it may reduce the risk of turning chronic organ dysfunction.4.The application of sequential blood purification therapy for poisoning MODS is more expensive and does not reduce hospitalization time.
