Objective:To compare the clinical efficacy of two kinds of treatment with arthroscopic on traumatic patellofemoral joint instability:1).lateral patellar retinacular loose solution,medial patellar retinacular reefing.2).lateral patellar retinacular loose solution with arthroscopic,medial patellar retinacular reefing and MPEL rebuilding.Methods:Through retrospectively analyzing clinical data of 48patients?48 knees?who has traumatic patellofemoral joint instability obtained from the department of joint motion in AffiliatedNanhua Hospital University of South Chinaduring January 2014 to June 2016.Each group of patients were carried out different surgical treatments with the assistance of arthroscopic.The general conditions of two surgical methods were compared including operation duration?min?,amount of bleeding?ml?,hospital stays?d?and hospitalization costs?yuan?.All patients were implemented follow-up Lysholm knee joint scale,IKDC scale and Kujala patellofemoral joint scale before and after surgery,and also taken patellar axial radiographs,LPA,PFI,LS and Q angle measured.The indicators of the two different surgical methods before and after surgery were compared,the scales and objective indicators of two surgical methods were analyzed statistically,and all scales were carried out statistical analysis after surgery.Through this way,the difference between the two surgeries were found.Results:48 cases?48 knees?after surgery were followed up successfully?100%?,and the follow-up time was 6 to 35 months,which means the average time was 17.25 months.Based on the final follow-up patellar axial radiographs by two surgical methods,the measurement of LPA,CA,PFI,LS and Q angle from the group of lateral patellar retinacular loose solution,medial patellar retinacular reefing with arthroscopic?Group A?was?9.25±1.9?o,?9.63±3.1?o,?0.92±0.22?,?3.20±1.0?mm,and?12.25±3.0?o respectively;These indicators from the group of combining lateral patellar retinacular loose solution with arthroscopic,medial patellar retinacular reefing with MPEL rebuilding under the arthroscopic?Group B?was?9.67±1.6?o,?9.13±3.6?o,?0.84±0.1?,?3.10±0.9?mm,and?10.83±2.6?o;Those mentioned above exhibited significantly difference compared to that before surgery?P<0.05?.The Lysholm knee joint scale,IKDC scale and KujalapatellofemoraljointscaleofGroupA?89.38±1.2?,?74.66±2.3?,?89.96±2.1?.These indicators of the final follow-up of Group B were?91.25±2.5?,?77.53±2.9?,?92.33±2.2?.The mentioned above exhibited significantly difference compared to that before surgery?P<0.05?.The final follow-up Lysholm knee joint scale,IKDC scale and Kujala patellofemoral joint scale presented significantly difference between lateral patellar retinacular loose solution,medial patellar retinacular reefing group and combining lateral patellar retinacular loose solution with arthroscopic,medial patellar retinacular reefing with MPEL rebuilding group under the arthroscopic?P<0.05?,Group B scored better than Group A.Conclusions:1.Treatment of traumatic patellofemoral joint instability by two different surgeries suggested the knee joint function of patients were obviously improving after surgeries.2.The clinical efficacy of lateral patellar retinacular loose solution with arthroscopic,medial patellar retinacular reefing with MPEL rebuilding to cure traumatic patellofemoral joint instability were superior to the lateral patellar retinacular loose solution,medial patellar retinacular reefing.
