Objective: To evaluate the efficacy and safety of berberine in the treatment for polycystic ovary syndrome. Methods: The randomized controlled trials of berberine in the treatment for polycystic ovary syndrome were searched from PubMed,EMbase,Cochrane Central Register of Controlled Trials(CCRCT),CBM,Wanfang datebase and CNKI.The randomized controlled trials were strictly determined according to the inclusion and exclusion criteria. We evaluated the quality of the included studies using revised Jadad's Scale, and then performed meta-analyses by using Rev Man 5.3 software. We evaluated the metabolic outcomes, sex hormone ralated outcomes and side effects. The metabolic outcomes including body mass index, waist to hip ratio, fasting blood glucose, fasting insulin levels, homeostasis model assessment of insulin resistance, blood lipids, sex hormone ralated outcomes including serum follicle stimulating hormone, leutinizing hormone, testosterone, sex hormone binding globulin and free testosterone index. Results: Our study included 6 randomized controlled trials involving 259 patients. The results of meta-analyses showed that berberine can significantly decrease waist to hip ratio(MD-0.04,95%CI(-0.06,-0.03);P<0.00001),fasting blood glucose(MD-0.33,95%CI(-0.52,-0.14);P=0.0006),fasting insulin levels(MD-1.46,95%CI(-1.79,-1.13);P<0.00001),homeostasis model assessment of insulin resistance(MD-1.45,95%CI(-2.09,-0.81);P<0.00001),total cholesterol(MD-0.55,95%CI(-0.73,-0.37);P<0.00001),triglycerides(MD-0.2,95%CI(-0.32,-0.09);P=0.0007),low-density lipoprotein cholesterol(MD-0.41,95%CI(-0.65,-0.16);P=0.001),serum testosterone(MD-0.79,95%CI(-1.36,-0.21);P=0.007),leutinizing hormone(MD-0.73,95%CI(-1.29,-0.18);P=0.009),Free testosterone index(MD-1.3,95%CI(-1.73,-0.88);P<0.00001);increase high-density lipoprotein cholesterol(MD0.11,95%CI(0.08,0.14);P < 0.00001)and Sex Hormone Binding Globulin(MD13.71,95% CI(8.92,18.5);P<0.00001)compared with that of control; There were no differences in body mass index(P=0.27),follicle stimulating hormone(P=0.54)and gastrointestinal symptoms rate(P=0.13)between the two groups. Conclusion: Berberine is effective and safe in the treatment on metabolic outcomes and hormone levels for polycystic ovary syndrome, and can be selected in the clinical treatment.
