| Objective To investigate the clinical significance of bronchoalveolar lavage(BAL)in treating acute bronchiolitis,and to provide a clinical basis for further improving the curative effect of bronchiolitis and promoting the recovery of the disease.Methods Afiberopticbronchoscopy(referred to as bronchoscopy)indications of acute bronchiolitis in children was selected as subjects from children's respiratory department of Chenzhou Hospital Affiliated to South China University from January 2015 to December 2016.According to whether use the bronchoscopy BAL or not,all the children were divided into lavage group and the controlgroup.The control group were received conventionaltreatmentincluding respiratory support,control of wheezing,maintenance of water and electrolyte environment stability and other treatment.The lavage group was treated with bronchoscopy BAL on the basis of conventional treatment.To analyze the clinical efficacy,changes of pulmonary function oftwo groups of children after treatment by different methods andto analyze safety of bronchoscopy.Results(1)A total of 95 patients with acute bronchiolitis were enrolled in the study,53 patients in the lavage group and 42 patients in the control group.There was no statistical difference in age,sex and body weight between the two groups(P> 0.05).(2)The time of disappearance of cough,wheezing,shortness of breath and lung rales of the lavage group was shorter than thoseof the control group(P <0.05).(3)We found no statistical differencebetween lavage group and control group after comparingthe pulmonary function before receiving treatment(P>0.05);The pulmonary function of both groups were improved after treatment(P<0.05);The pulmonary function of the lavage group was improved significantly compared with the control groupafter treatment(P <0.05).(4)The improvement rate of lung CT of the lavage group was higher than that of the control group at the 7th day(P <0.05).(5)The average duration of hospital stay of the lavage group was shorter than that of the control group(P <0.01);The average hospital costsof the lavage group was shorter than that of the control group(P <0.01).(6)In the lavage group,only a few cases showed a transient decrease in blood oxygen saturation,epistaxis,and other slight complication.There was no serious complication in the lavage group.Conclusions Bronchoscopy BAL treatment of acute bronchiolitis can significantly improve clinical efficacy,improve lung function,promote disease recovery,while bronchoscopy BAL is a safe and reliable technology for diagnosis and treatment. |
