Clinical Observation Of Bronchoalveolar Lavage Through Fiber Bronchoscopy In The Prevention And Treatment Of Severe Burn Patients With Pulmonary Infection

Objective:To investigate its clinical value and curative effect of bronchoalveolar lavage through fiber bronchoscopy in the prevention and treatment of severe burn patients with pulmonary infection.Methods:104 patients recurrently diagnosed with severe burn in our hospital were involved in this research. They were randomly and evenly divided into two groups. Control group was given the conventional treatment. Observation group was additionally given bronchoalveolar lavage through fiber bronchoscopy. Respiratory rate measured as contrast(RR), heart rate(HR) and arterial blood gas analysis the two groups of patients before treatment and 2 weeks and blood WBC, TNF-α, IL-6 levels were compared in patients with lung infection rate, time of infection control and infection control rate.Result:(1) Pre-treatment control group and observation group, RR and HR had no significant difference(P> 0.05), after treatment in the control group and the observation group, RR and HR are declining, but the control group, RR and HR lower than the observation group(P <0.05);(2) Before treatment of arterial blood gas p H, Pa O2, Pa CO2 indicators showed no significant difference(P> 0.05), the observation group was significantly higher in patients with arterial blood gas p H, Pa O2 index than the control group after treatment(P <0.01), significantly decreased arterial blood gas Pa CO2 index than the control group(P <0.01);(3) Before treatment, patients with venous WBC level difference was notstatistically significant(P> 0.05), after treatment, the observation group were intravenous WBC level significantly lower than the control group(P <0.01);(4) Pre-treatment control group and observation group TNF-α, IL-6 levels were not significantly different(P> 0.05), after treatment in the control group and the observation group TNF-α, IL-6 levels were declining, but the control group TNF-α,IL-6 levels lower than the observation group(P <0.01).(5) Lung infection in patients in the observation group than the control group decreased significantly, infection control time significantly shorter than the control group, a significant increase compared with the rate of infection control group, the differences were statistically significant(P <0.01).(6) The observation group total effective rate was significantly higher, the difference was statistically significant(P <0.05).Conclusion:Bronchoalveolar lavage through fiber bronchoscopy plays a positive effect in the prevention and treatment of severe burn patients with pulmonary infection.Effectively improve lung function and the body state of infection in patients,recommend clinical popularization and application.
