Objective To investigate the epidemiological characteristics and chronological change of deaths in PICU of Chenzhou First People's Hospital affiliated to South China University.Methods We retrospectively analyzed the clinical data of deaths from January 2007 to December 2016.We divided the enrolled patients into groups according to residence(urban and rural),gender(male and female),age(infant from 29 days to 1 year old,child from 1 to 3 year old,preschool from 3 to 7 year old and school from 7 to 14 year old)and hospital stay(? 1 day,-3 days,-7 days,>7 days),Comparative analysis was conducted between groups in terms of mortality,major causes of death,pediatric critical score and prognosis.Results(1)There were a total of 8532 cases of critically ill children in PICU from 2007 to 2016 with a mortality of 5.74%;Mortality in the first five years are higher than that of the last five years of mortality.The difference is statistically significant(p<0.05).(2)The infant group had the most cases of death,with the infant group,children group,preschool group,school age group accounting for 53.1%,27.3%,13.5% and 6.1%of total deaths,respectively.The number of deaths is mostly in male patients(62.0%),in rural areas(70.0%),and the highest number of deaths is in January(15.1%).(3)The major cause of death disease are severe pneumonia(32.9%),severe encephalitis(27.3%)and severe sepsis(15.5%).Infant group are mainly severe pneumonia(accounting for48.8%),while children group,preschool group and school age group are mainly severe encephalitis with a rate of 47.0%,37.9% and 46.7%respectively.in the proportion of patients with hospital stays ?1 day,-3days,-7 days,>7 days are 30.0%,23.0%,17.6% and 29.4% respectively,among which the first two groups are mainly severe pneumonia(36.1%and 32.7%),while the latter two groups are mainly severe encephalitis(31.4% and 37.5%).(4)The mortality of critical children in non-risk group,risk group and high risk group are respectively 1.2%,2.7% and19.5%.The difference is statistically significant(p<0.05).Conclusions The overall mortality in PICU of Chenzhou Children's Hospital is declining,in which the highest number of deaths are infant,and the largest one are male,mainly in rural areas,and January has highest mortality.Severe pneumonia,severe encephalitis,and severe sepsis are major causes of death.In infant group severe pneumonia is the major cause of death,and in children group,preschool group and school age group,the major causes of death are mainly severe encephalitis.Patients with a hospital stay less than 3 days mainly die of severe pneumonia,while the rest of patients mainly die of severe encephalitis.Patients with lower critical score and poorer status tended to have higher mortality.
